Backg ound: Mul iple-choice ques ions (MCQs) a e widely used in medical
educa ion due o hei e iciency in assessing a b oad ange o knowledge. Howe e ,
adi ional g ading o MCQs can be labo -in ensi e and p one o human e o ,
especially in la ge olumes. Wi h inc easing class sizes, he e is a g owing need o
mo e e icien and accu a e g ading sys ems.
Objec i e: This s udy e alua es he e ec i eness o in eg a ing oice- o- ex
echnology (VTT) and Excel au oma ion o enhance he e iciency and accu acy o
MCQ g ading in medical educa ion.
Me hods: A o al o 3,000 simula ed MCQs esponses we e e alua ed using bo h
manual and au oma ed me hods. VTT echnology was used o ansc ibe esponses,
which we e hen o ganized in o a able using a La ge Language Model (LLM) o
da a pa sing and s uc u ing and sco ed in Excel using au oma ion echniques. Two
e alua o s assessed he pape s, and ime aken, e o a es, e alua o a igue, and
sa is ac ion we e compa ed be ween me hods.
Resul s: The au oma ed me hod signi ican ly educed e alua ion ime o 45.9
minu es (0.76 man-hou s) compa ed o 194.82 minu es (3.25 man-hou s) o manual
e alua ion, wi h a simila high accu acy a e o 99.96% o bo h me hods. E alua o
a igue was lowe , and sa is ac ion was highe wi h he au oma ed me hod.
Conclusion: The in eg a ion o VTT echnology and Excel au oma ion signi ican ly
imp o es he e iciency o MCQ g ading while main aining high accu acy. This
app oach o e s a scalable, cos -e ec i e solu ion o medical educa ion se ings,
pa icula ly in esou ce-limi ed en i onmen s. Fu u e esea ch could expand his
me hod o o he ypes o assessmen s and explo e addi ional au oma ion ools o
u he enhance educa ional e alua ion p ocesses.

INTRODUCTION
Mul iple-choice ques ions (MCQs) a e a undamen al componen o assessmen in medical educa ion, widely used o hei
abili y o e icien ly es a b oad ange o knowledge (Haladyna, Downing, & Rod iguez, 2002). Despi e hei popula i y,
g ading MCQs, especially in la ge olumes, can be labo -in ensi e and suscep ible o human e o , po en ially impac ing
he eliabili y and alidi y o assessmen s (Schuwi h & an de Vleu en, 2011). Wi h inc easing class sizes and he need
o equen assessmen s, he e is a g owing demand o mo e e icien and accu a e g ading sys ems in medical educa ion
(Case & Swanson, 2002).

Excel au oma ion ha e shown p omise in educing he wo kload associa ed wi h g ading while main aining, i no
enhancing, accu acy (Pea , 2000). Voice- o- ex echnology o e s a hands- ee way o en e da a, which, when combined
wi h Excel’s powe ul au oma ion capabili ies, can signi ican ly s eamline he g ading p ocess (Tobin, 1998). This s udy
explo es he e ec i eness o in eg a ing oice- o- ex and Excel au oma ion in he e alua ion o MCQs in a medical
educa ion se ing. The goal is o compa e he e iciency, accu acy, and e alua o sa is ac ion be ween adi ional manual
g ading and a echnologically enhanced au oma ed app oach.
Me hods
MCQ E alua ion P ocess
The s udy e alua ed a o al o 3,000 MCQs simula ing he heo y assessmen s o i s -yea MBBS s uden s in Physiology.
The MCQs we e dis ibu ed ac oss 300 exam shee s, wi h each shee con aining 10 MCQs, each ha ing ou op ions (A,
B, C, D). These exam shee s we e u he di ided in o 15 bundles, each comp ising 20 pape s. Each bundle was imed
sepa a ely o assess he p ocessing e iciency o he di e en e alua ion me hods—Voice- o-Tex (VTT) and manual.
As shown in Figu e 1, Du ing he VTT me hod, he e alua o ansc ibed each MCQ's esponse by e bally indica ing he
op ions as "Alpha," "B a o," "Cha lie," and "Del a" o a oid con usion du ing he oice- o- ex con e sion p ocess. The
se ial numbe s o he MCQ pape s we e ead sequen ially, ollowed by he chosen op ions o each ques ion. I a ques ion
was le unanswe ed o had mul iple answe s, he e m "Fun" was spoken. The VTT me hod was conduc ed i s o all 300
shee s o ansc ibe esponses and calcula e sco es using au oma ed p ocesses. A e comple ing he VTT e alua ion o all
shee s, he manual me hod was hen employed. Du ing he manual e alua ion, he o al sco e o each shee was eco ded
di ec ly on he shee a e sco ing all 10 MCQs, and his sco e was included in he p ocessing ime o each manual
e alua ion. Two e alua o s pa icipa ed in he assessmen s: E alua o 1 p ocessed se en bundles, while E alua o 2 handled
he emaining eigh bundles.
Technology and Tools Used
The eco ded ime o each me hod ep esen ed he du a ion equi ed o p ocess he MCQ shee s pe bundle. Fo he VTT
me hod, p ocessing ime included ansc ibing he esponses using Google Docs' oice- o- ex ea u e, se o "English
(Uni ed Kingdom)" wi h he "English (India)" a ian o ma ch he e alua o s' accen s. The in-buil lap op mic ophone was
used wi h op imized se ings o accu a e inpu . Dic a ion was conduc ed a a mode a e pace o cap u e each wo d co ec ly,
as speaking oo quickly o en led o incomple e o inaccu a e esponses. B ie pauses we e made be ween dic a ing answe s
o allow he so wa e ime o p ocess he inpu and educe e o s. A s opwa ch ea u e on an And oid phone was used o

dis ac ions. All o he backg ound applica ions on he compu e we e closed o educe sys em noise and enhance he
pe o mance o Google Docs' oice- o- ex ea u e. These s eps we e aken o ensu e op imal ansc ip ion accu acy.
E o -Checking P ocedu es
Real-Time Moni o ing: Real- ime moni o ing was employed du ing bo h VTT and manual me hods o ensu e accu acy.
Fo he VTT me hod, e alua o s used keyboa d sho cu s o quick co ec ions du ing dic a ion (e.g., C l + Z o undo, C l
+ Dele e o dele e he las wo d, En e o sepa a e new en ies). Au oma ed e o de ec ion ea u es, such as spell check and
g amma ools in Google Docs, we e disabled o p e en unnecessa y lagging o non-s anda d en ies as e o s. In he
manual me hod, e alua o s c oss-checked hei wo k con inuously o minimize human e o s due o o e sigh o a igue.
Pos -Dic a ion Re iew: A e p ocessing each bundle o MCQ shee s, addi ional e o -checking was pe o med. In he
VTT me hod, he ansc ibed esponses we e in e mi en ly compa ed agains he o iginal answe shee o key o e i y
accu acy and make necessa y adjus men s be o e o ganizing he da a in o a able using LLM o Excel sco ing. Fo he

Co ela ion analysis e ealed a weak nega i e co ela ion o he Manual me hod (-0.2107) and a mode a e nega i e
co ela ion o he VTT me hod (-0.5391), indica ing ha ime in es men in VTT may educe e o s mo e e ec i ely, as
de ailed in Table 2.
Resul s
The s udy e alua ed 3,000 mul iple-choice ques ions (MCQs) using bo h manual and au oma ed me hods. The au oma ed
me hod (Voice- o-Tex , VTT) signi ican ly educed e alua ion ime o 45.9 minu es (0.76 man-hou s) compa ed o 194.82
minu es (3.25 man-hou s) o manual e alua ion, esul ing in an a e age ime pe pape o 9.18 seconds o VTT e sus
38.96 seconds o he manual me hod. Figu e 3 illus a es he subs an ial ime sa ings achie ed when using he VTT
me hod o MCQ e alua ion compa ed o he manual me hod, clea ly showing he e iciency ad an age o he VTT
app oach.
In e ms o e o s, he manual me hod esul ed in 13 e o s, leading o an e o a e o 0.043 pe pape and an accu acy o
99.96%, while he au oma ed me hod had 12 e o s, esul ing in an e o a e o 0.04 pe pape and an accu acy o 99.96%.

as inco ec o ice e sa, e en when o he esponses in he sequence we e accu a ely ma ked (Figu e 7) and Table 3. This
aligns wi h indings om p e ious s udies ha emphasize human e o in manual g ading due o a igue o dis ac ion
(Haladyna, Downing, & Rod iguez, 2002). In con as , e o s in he Voice- o-Tex (VTT) me hod we e p ima ily echnical,
such as incomple e eco dings whe e only 9 esponses we e cap u ed ins ead o 10, as depic ed in Figu e 6 and Table 3.

The use o mul iple-choice ques ions (MCQs) is a s anda d p ac ice in heo y examina ions ac oss all subjec s in medical
colleges (Schuwi h & an de Vleu en, 2011). Howe e , many ins i u ions ace limi a ions such as insu icien s a ing
and lack o ad anced g ading echnologies like Op ical Ma k Recogni ion (OMR) shee s and scanne s. This is pa icula ly
challenging when he numbe o MCQ shee s pe subjec anges om 200 o 500, as shown in Table 1 and Figu e 5, which
illus a e he ex ensi e ime and e o a es associa ed wi h manual g ading me hods.. The au oma ed Voice- o-Tex and
Excel me hod o e s a p ac ical and cos -e ec i e al e na i e, equi ing minimal in as uc u e, making i highly accessible
e en in esou ce-cons ained se ings. This app oach also allows o he digi al sha ing o ma ked esponses, enhancing
anspa ency and p o iding a aluable esou ce o pos -examina ion e iew. The adop ion o such echnology can alle ia e

hough his is unlikely o signi ican ly impac he o e all indings. While e alua o s we e awa e o being imed, which
migh ha e in oduced a sligh bias, hei na u al pace can be main ained wi h minimal in luence. Technical challenges,
such as occasional dic a ion e o s and Excel’s limi a ions, a e manageable and do no signi ican ly hinde he b oade
