Rele ance: non-small cell lung cance (NSCLC) emains one o he leading causes o cance -

Pu pose o he s udy: o conduc a sys ema ic e iew and me a-analysis o clinical ials
compa ing he e icacy and ole abili y o a ge ed he apies and immuno he apeu ic agen s in
NSCLC.
Ma e ials and me hods: a li e a u e sea ch was pe o med in PubMed, Embase, and he
Coch ane Lib a y o he pe iod 2015–2024. Eligible s udies included andomized clinical ials and
me a-analyses epo ing o e all su i al (OS), p og ession- ee su i al (PFS), objec i e esponse
a e (ORR), and ad e se e en p o iles. Pooled es ima es we e calcula ed using a andom-e ec s
model, and in e s udy he e ogenei y was assessed wi h he I² s a is ic.
Resul s: а o al o 36 s udies, including o e 14,000 pa ien s, we e analyzed. In pa ien s wi h
d i e mu a ions, a ge ed he apies signi ican ly imp o ed PFS (HR = 0.48; 95% CI: 0.41–0.56) and
inc eased ORR up o 65%, ma kedly ou pe o ming chemo he apy. Immuno he apy wi h PD-1/PD-
L1 inhibi o s demons a ed a clinically meaning ul OS bene i in pa ien s wi hou EGFR/ALK
mu a ions and wi h high PD-L1 exp ession (≥50%) (HR = 0.74; 95% CI: 0.66–0.83). Sa e y analysis

Conclusions: This me a-analysis suppo s he p e e en ial use o a ge ed he apies in pa ien s
wi h de ined molecula al e a ions and highligh s he e icacy o immuno he apy in coho s wi h high
PD-L1 exp ession and no d i e mu a ions. Op imal he apeu ic s a egies should be guided by
molecula p o iling and bioma ke assessmen . Combina ion egimens in ol ing a ge ed and
immuno he apeu ic agen s ep esen a p omising di ec ion, wa an ing u he mul icen e ials and
pha macoeconomic e alua ion wi hin na ional heal hca e sys ems.
