Acute appendicitis – clinical symptoms and scoring systems

Abstract

Acute appendicitis is the most common cause of acute abdominal surgery, with a peak incidence between 10 and 30 years of age and an annual prevalence of approximately 100 per 100,000 people. Despite well-established diagnostic and therapeutic algorithms, early diagnosis remains challenging, especially in children, elderly patients, pregnant women, and those with atypical appendix locations. The disease follows a dynamic inflammatory progression with three main stages—catarrhal, phlegmonous, and gangrenous—each associated with characteristic clinical and morphological features.The clinical diagnosis is based on symptom chronology, with migratory pain, anorexia, nausea, and localized tenderness in the right iliac fossa as key indicators. Numerous classical physical signs (Blumberg, Rovsing, Psoas, Obturator, Sitkovski, etc.) remain useful in clinical evaluation. However, no single clinical or laboratory marker is sufficiently specific; hence, scoring systems have become integral to modern diagnostic algorithms.Among the most widely used models, the Alvarado and Appendicitis Inflammatory Response (AIR) scores show the best combination of sensitivity and specificity. Pediatric populations benefit from the PAS, while newer tools such as RIPASA, MZXBTCH, and Sammalkorpi scores offer improved stratification of complicated appendicitis. A balanced approach that integrates clinical judgment, validated scoring systems, and selective imaging (ultrasound or CT) provides optimal diagnostic accuracy and reduces unnecessary surgeries.

Въпреки добре установените диагностични и терапевтични алгоритми, ранната диагноза остава предизвикателство, особено при деца, възрастни пациенти, бременни жени и при атипично разположение на апендикса. Заболяването протича като динамичен възпалителен процес с три основни стадия – катарален, флегмонозен и гангренозен – всеки със специфични клинични и морфологични характеристики. Клиничната диагноза се базира на хронологията на симптомите, като ключови белези са мигриращата болка, анорексията, гаденето и локалната болезненост в дясна хълбочна ямка. Множество класически физикални признаци (Блумберг, Ровзинг, Псоас, Обтуратор, Ситковски и др.) запазват своето значение в диагностиката. Нито един клиничен или лабораторен показател обаче не притежава достатъчна специфичност, поради което скоринговите системи се утвърждават като важен елемент на съвременните диагностични алгоритми. Сред най-широко използваните модели Alvarado и Appendicitis Inflammatory Response (AIR) демонстрират най-добро съчетание между чувствителност и специфичност. При деца се прилага PAS, а по-нови модели като RIPASA, MZXBTCH и Sammalkorpi предлагат по-точна стратификация на усложнените случаи. Оптималният диагностичен подход включва интеграция между клиничната преценка, валидирани скорингови системи и селективно използване на образни методи (ехография, КТ), което намалява честотата на ненужните операции и повишава точността на диагнозата. КЛЮЧОВИ ДУМИ остър апендицит, клинична диагноза, скорингови системи, Alvarado score, AIR score ВЪВЕДЕНИЕ Острият апендицит е най-честата хирургична спешност в корема с пик на честота между 10 и 30 години и годишна заболеваемост от 96–100 на 100 000 души [1,3]. Клиничната диагноза е трудна, особено при деца, възрастни хора, бременни жени и пациенти с атипично разположен апендикс. Приблизително 20–33% от случаите се представят с нетипични симптоми [3,7]. ДИАГНОСТИКА В развитието на острия апендицит, въпреки неговото разнообразно и динамично протичане се наблюдава известна стадийност на възпалителният процес, определящ се от морфологичните промени в апендикса, локализацията му, общото състояние на организма и настъпилите по-късно усложнения. В I стадий (8-18 ч.) морфологичните промени следствие на обструкцията на апендикуларния лумен довеждат до възникване на първичен афект в лигавицата на апендикса. Анамнестичните данни в този стадий дават информация за известни неспецифични продроми на заболяването: отпадналост, неразположение, субфебрилитет. Малко по- СП. ХИРУРГИЯ • J. Болка + поне един от последните 3 симптома в продължение на няколко часа се регистрира в около 90% от случаите. В случай, че никой от тези 3 симптома не е налице – вероятността за възникнал апендицит е малка. В този стадий диагнозата може само да се подозира! Тук се допуска и основната грешка при поставянето на диагнозата. Непознаването на началната симптоматика на класическата клинична картина на острия апендицит води до подценяване и неглижиране на оплакванията на болния и закъсняване на своевременната диагностика. Назначаването на симптоматично лекарствено лечение допринася за забавяне в поставянето на правилната диагноза. Във II стадий (12-24 ч.) възпалителният процес е обхванал значителна част от апендикса и е с тенденция да се прехвърля и върху париеталния перитонеум и периапендикуларните тъкани. Тук клиничната картина вече напълно се разгръща, и това е стадият, в който се поставя и диагнозата. III стадий настъпва след 24тия час, като при него се наблюдават усложненията, които могат да настъпят при остър апендицит – перфорация, дифузен перитонит, периапендикуларен инфилтрат и периапендикуларен абсцес. В този стадий пациентите са в увредено общо състояние, с изострени черти на лицето и могат да се наблюдават прояви на SIRS/ Сепсис. Основната причина болният да се обърне към лекар е болката в корема. Обикновено тя възниква внезапно, при пълно здраве, без значение ден или нощ. В част от болните тя е постоянна, при други с обхващане на нови зони на възпалителния процес - на тласъци, но не отзвучава. Болка с коликообразен характер е сравнително по-рядко срещана. Причина за възникване на болката се счита въвличането във възпалителния процес на силно чувствителният париетален перитонеум и мезото на апендикса. Обтурацията на просвета на апендикса от феколити и възникналият оток допълнително допринасят за подтискането на перисталтиката и засилват интензитета на болката. Първоначалната локализация на болката (при най-често срещаната илеоцекална локализация) е различна. В част от болните тя възниква в илеоингвиналната област, в друга започва в епигастриума (симптом на Kocher), при трети е дифузна в целия корем. При последните два варианта неопитният лекар може да бъде заблуден и да се насочи към стомашна или чревна патология. Окончателната локализация на болката при типично разположен апендикс, независимо как е започнала винаги е в дясната илеоингвинална зона в рамките на 24 часа. Интензитетът на болката е различен. В едни случаи тя е умерена, не предизвикваща някакви допълнителни промени в начина на живот. В други обаче, може да е по-силно изразена, но почти казуистични са случаите, когато тя е непоносимо силна. При болшинството болни тя е умерено силна по интензитет. Продължителността на болката варира в доста широк диапазон от минути до часове. Понякога тя може да затихне и въобще да не се появи отново, но в някои случаи „облегчение“ се наблюдава и при перфорация на възпаленият вече участък на апендикса. За истинско такова може да се приеме изчезването на другите симптоми на острия апендицит, и като цяло подобряване общото състояние на болния. При част от болните в положение на покой симтоматиката отзвучава, но при раздвижване на болния тя отново се възобновява. СП. ХИРУРГИЯ • J. Тези симптоми не са патогномонични за острия апендицит, а се регистрират и при други възпалителни интаабдоминално разположени органи. Обикновено повръщането се наблюдава в първите часове и то е еднократно. Многократното повръщане не е характерно за развитието на процеса в I-ви и II-ри стадий, но е често срещано в последния III стадий. Отсъствието на повръщане не трябва да се преценява като лека форма на апендицит, тъй като в редица случаи независимо от тежкото протичане на заболяването този симптом не се регистрира. За острия апендицит е характерно времето за появата на повръщането. Наличието му в първите часове на заболяването дори без да се регистрира болка в илеоингвиналната област може да насочи лекаря в посока остър апендицит. Тези два симптома се срещат по-често при деструктивните форми на апендицит. Апетитът при острия апендицит е намален в лека до умерена степен. Пасажът поне в началото на този стадий също е без изменения, но при преминаването му към третия стадий се наблюдава известна пареза чревния тракт. ИЗСЛЕДВАНЕ НА БОЛНИЯ И ДИАГНОСТИЧНА ЦЕННОСТ НА РАЗЛИЧНИТЕ СИМПТОМИ Физикални методи Оглед Общото състояние при болшинството болни е добро. Понякога дори при при деструктивен апендицит общото състояние страда слабо. С течение на времето при нарастване на възпалителните изменения състоянието на болните постоянно се влошава. В повечето случаи болните предпочитат да лежат на гръб, по-рядко на десния хълбок, понякога с флексия в колянните стави. Обикновено положението на лявата страна се избягва, тъй като цекумът, макар и добре фиксиран, се увлича и натежава, а интралуменните газове се преместват към цьокума и раздувайки го засилват болката. Езикът „огледалото“ на корема според Хипократ, в началото е леко обложен, макар и все още влажен, но почти при всички болни е налице промяна. С напредване на възпалителния процес той-става сух, налепите постепенно придобиват жълтокафява окраска. Пулсът по правило е леко към умерено учестен. Температурната реакция е с някои особености. Обикновено температурата е субфебрилна с тенденция за бавно покачване. Не са малко и случаите където тя е нормална. Дори и при деструктивни форми не е изключение да не се регистрира фебрилитет. Висока температура над 39-39,5° е рядко изключение. По-съществено значение за диагностиката има измерването на по-висока от 1° температурна разлика между аксиларна и интраректална температура. Палпациятa е методът, предоставящ най-убедителните данни за развитието на остър апендицит. Изследването на корема трябва да се проведе внимателно, започва се първоначално с повърхностна и по-късно преминаваща в дълбока палпация първоначално от лява и завършваща в дясна илеоингвинална област. За диагностиката на острия апендицит са предложени редица болеви симптоми. Механизмът за възникване на болшинството от тях e обусловен от повишената СП. ХИРУРГИЯ • J. Най – често срещаният и най-важният болеви симптом е наличието на мускулна резистентност. Мускулната резистентност може да не бъде така открояваща се при болни с добре изразена подкожна мастна тъкан или при болни в напреднала и старческа възраст, но при всички случаи е налична. Наличието на положителен Blumberg в илеоцекалната област (който не е патогномоничен за остър апендицит) окончателно определя преминаването към оперативно лечение при болния. Степента му на изразеност зависи от степента на възпаление и разположението му в близост до париеталния перитонеум. При затруднение в диагностичния процес могат да се използват и други прийоми за изследване на болния. Бърз метод за установяване на локализирана болезненост. Shchetkin– Blumberg Рязка болка при бързо отнемане на ръката след дълбока палпация. Дълбока бавна палпация, последвана от рязко освобождаване . Силно изразено перитонеалн о дразнене. Класически симптом, често използван в практиката. Kummell Болка при рязко освобождаване на натиск след дълбока палпация (вариант на Blumberg). Бавно притискане и рязко отдръпване на ръката. Внезапно разтягане на париеталния перитонеум. Подчертава перитонеалнот о дразнене. СП. ХИРУРГИЯ • J. Пациентът пада на пети от леко повдигнато положение. Ударната вълна се предава към перитонеума. Бърз тест за перитонеално дразнене. Sitkovski (Rosenstein) Болката в дясната илиачна област се усилва при легнало положение на лявата страна. Пациентът ляга наляво, наблюдава се реакцията. Придвижване на цекума и апендикса дразни перитонеума. Класически признак за апендицит. Rovsing Натиск в лявата илиачна област предизвиква болка вдясно. Дълбока палпация вляво с избутване на съдържимото нагоре по колон. Придвижване на газове раздува цекума и дразни апендикса. Помага при диагнозата, особено при перитонеално дразнене. Модифицира н Rovsing Болка вдясно при бързо издигане от легнало в седнало положение с натиск вляво. Комбиниране на натиск и динамично движение. Повишено напрежение върху цекума. По-рядко използван. Думбадзе Болка в дясната илиачна област при натиск върху предната ректална стена. Ректално туширане с внимателна палпация напред. Дразнене на перитонеума при тазов апендикс. Важен при атипични тазови случаи. Kulenkampff Болка при палпация на дясната стена на ректума. Ректална палпация вдясно. Дразнене на възпалени тазови структури. Полезен при тазов апендикс. Ten Horn Болка в дясната илиачна област при издърпване на семенния корд. Леко издърпване надолу на funiculus spermaticus. Напрежение, пренасяно към апендикса. Характерен при мъжки пациенти. Horn Болка при повдигане на тестиса и семенния корд вдясно. Леко издърпване нагоре. Напрежение, предавано по семенния канал. Подобен на Ten Horn. Аускултация: Аускултацията е със слаб принос в диагностиката на този етап. Обикновено се установява нормална или леко забавена перисталтика. израз на съпътстваща чревна пареза. Клинични симптоми Типичната клинична картина включва: • болка, започваща периумбиликално и мигрираща към дясната илиачна ямка, • безапетитие • гадене и/или повръщане, • субфебрилитет или фебрилитет (до 38–38.5°C), • локална болка при палпация в точката на McBurney, • мускулна резистентност, положителен симптом на Blumberg (rebound tenderness) и положителни специфични знаци (Rovsing, psoas, obturator) [3,7]. Мета-анализ показва, че най-чувствителни симптоми са миграция на болката (чувствителност 0.64–0.77), болка в дясната илиачна ямка (0.81), безапетитие (0.79) и левкоцитоза [3,4]. При деца и възрастни над 65 г. диагнозата често се забавя поради атипични прояви, водещо до повишен риск от перфорация и усложнения [3,5]. Скринингови системи и клинични скоринг модели Поради липсата на специфичен лабораторен маркер или образен метод със 100% диагностична стойност, в практиката се използват скринингови алгоритми с цел подобряване на диагностичната точност. 1. Alvarado Score (1986) Състои се от 8 критерия – симптоми, обективни находки и лабораторни показатели – с максимален брой точки 10. Критерии – миграция на болката, анорексия (ацетон в урината), гадене/повръщане, мускулна резистентност в долен десен коремен квадрант (2), положителен симптом на Blumberg, повишена телесна температура, левкоцитоза (2), олевяване, повишени левкоцити в урината, болка при РТ. • <5 точки: малка вероятност • 5–6: наблюдение/образна диагностика • ≥7: вероятен апендицит – препоръчва се хирургично поведение [6] Системата е широко използвана, но има по-ниска точност при жени и деца. . Modified Alvarado Score (MASS) Полезен на места, където не се изследва олевяване. Наличието на допълнителна клинична находка в някои проучвания показа 68% сензитивност и 58% специфичност. 3. Appendicitis Inflammatory Response (AIR) Score (2008) Включва CRP и степен на неутрофилия, които подобряват точността. Разделя пациентите в 3 рискови групи: • нисък (0–4 т.), • среден (5–8 т.), • висок риск (9–12 т.). AIR Score има по-добра диагностична стойност спрямо Alvarado (AUC 0.89–0.97) [2]. СП. ХИРУРГИЯ • J. Clinical diagnosis remains challenging, particularly in children, the elderly, pregnant women, and patients with atypically located appendices. Approximately 20– 33% of cases present with atypical symptoms [3,7]. DIAGNOSIS Despite its variable and dynamic course, acute appendicitis follows a specific staging pattern of the inflammatory process, determined by the morphological changes in the appendix, its anatomical position, the general condition of the patient, and the occurrence of subsequent complications. In Stage I (8–18 hours), morphological changes resulting from luminal obstruction of the appendix lead to the development of a primary inflammatory focus in the mucosa. The clinical history at this stage often includes nonspecific prodromal symptoms such as malaise, fatigue, and low-grade fever. Soon after, visceral-type abdominal pain develops, accompanied by anorexia, nausea, and occasionally by single vomiting episodes. Pain associated with at least one of these three symptoms, lasting several hours, occurs in approximately 90% of cases. If none of these symptoms are present, the likelihood of appendicitis is low. At this stage, the diagnosis can only be suspected. Misinterpretation of early symptoms is the most common diagnostic error, leading to delayed recognition. Symptomatic medical treatment at this point often contributes to diagnostic delay. СП. ХИРУРГИЯ • J. The clinical picture becomes fully developed, and this is the stage when the diagnosis is most often established. Stage III occurs after 24 hours and includes complications such as perforation, diffuse peritonitis, periappendiceal infiltrate, and abscess formation. At this stage, patients present in a compromised general condition, with sharp facial features and signs of systemic inflammatory response syndrome (SIRS) or sepsis. The main reason for seeking medical attention is abdominal pain. Typically, the pain appears suddenly in an otherwise healthy patient, irrespective of the time of day. In some cases, the pain is constant; in others, it progresses in waves as the inflammatory process spreads, but it rarely subsides completely. Colicky pain is less common. The pain arises from irritation of the highly sensitive parietal peritoneum and mesoappendix. Luminal obstruction by fecaliths and subsequent edema contribute to decreased peristalsis and intensify pain severity. The initial localization of the pain varies with the position of the appendix (most commonly ileocecal). It may begin in the right lower quadrant, epigastrium (Kocher’s sign), or diffusely across the abdomen. The latter two patterns may mislead an inexperienced clinician toward gastric or intestinal pathology. Ultimately, regardless of its onset, the pain localizes to the right iliac fossa within 24 hours. Pain intensity also varies: it may be mild and tolerable or, in some cases, more severe, though excruciating pain is extremely rare. For most patients, pain is moderate in intensity. The duration of pain ranges widely, from minutes to hours. It may temporarily subside but recur, particularly after perforation of the inflamed appendix. Apparent “relief” should not be misinterpreted unless accompanied by improvement in general condition and resolution of other symptoms. Nausea and vomiting are common but not constant features. They are not pathognomonic for appendicitis and may occur with other intra-abdominal inflammatory processes. Vomiting typically appears early and is often limited to one episode. Repeated vomiting is uncommon in the first two stages but frequent in Stage III. Absence of vomiting does not exclude severe appendicitis, as even destructive forms may progress without it. The timing of vomiting is diagnostically significant—its occurrence in the early hours, even before localized pain develops, can suggest acute appendicitis. These symptoms are more common in destructive forms of the disease. Appetite is usually mildly to moderately reduced. Bowel movements are initially preserved but become impaired as the process advances to Stage III, where partial intestinal paresis may occur. EXAMINATION OF THE PATIENT AND DIAGNOSTIC VALUE OF VARIOUS SYMPTOMS Physical Examination Inspection: In most patients, the general condition is satisfactory. Occasionally, even in destructive appendicitis, the overall condition is only mildly affected. As the inflammatory process progresses, the patient’s condition gradually deteriorates. Patients typically prefer to lie on their back, less often on the right side, sometimes with knees flexed. The left lateral position is usually avoided because the cecum, though well fixed, tends to shift and become heavy, while intraluminal gas moves toward it, increasing distension and pain. СП. The pulse is usually mildly to moderately accelerated. Temperature tends to be subfebrile with slow elevation; regular readings are not uncommon. Even in destructive forms, fever may be absent. Temperatures above 39–39.5°C are rare. A diagnostic clue is a difference greater than 1°C between axillary and rectal temperatures. Palpation provides the most convincing evidence of acute appendicitis. The abdominal examination must be conducted carefully, beginning with light palpation and proceeding to deep palpation, starting from the left and ending in the right iliac fossa. Many pain-related signs have been described, most arising from localized peritoneal irritation in the area of the appendix. The most consistent and essential finding is muscular guarding. It may be less pronounced in obese patients or the elderly, but it is virtually always present. A positive Blumberg sign in the ileocecal region—though not pathognomonic—typically indicates the need for surgical intervention. The degree of rebound tenderness correlates with the extent of inflammation and proximity to the parietal peritoneum. Kummell sign Pain after sudden release of deep pressure (variant of Blumberg). Rapid stretching of the parietal peritoneum. Indicates peritoneal irritation. Markle (heel drop) sign Pain in the right iliac region upon sudden heel strike or jump. Shock wave transmitted to the peritoneum. Rapid test for peritoneal inflammation. Modified Rovsing sign Pain occurs when rising quickly from supine to sitting position while pressing on the left abdomen. Added cecal tension. Less frequently used. Bartomier– Michelson sign Pain increases when palpating the right iliac region while the patient lies on the left side. Forward displacement of the cecum increases contact with the examiner’s hand. Similar to the Sitkovski sign but with different mechanics. Cope’s psoas sign Pain on passive extension of the right hip while the patient lies on the left side. Stretching of the psoas muscle irritates the retrocecal appendix. Classic indicator of retrocecal appendicitis. Baldwin sign Pain in the right iliac fossa when raising the right leg while prone. Iliopsoas tension irritates the appendix. A variant of the psoas test. Obraztsov sign Pain during the resisted elevation of the straight right leg under the examiner’s pressure. Iliopsoas tension against the examiner’s hand irritates the appendix. Useful for retrocecal localization. Massouh sign Sliding fingers from the xiphoid downward and laterally—pain only on movement to the right. Local peritoneal irritation above the inflamed appendix. Rare but specific additional sign. Aaron sign Pressure over McBurney’s point causes referred pain in the epigastrium. Viscerosomatic reflex. Supportive diagnostic indicator. Dumbadze sign Pain in the right iliac fossa during rectal examination with anterior pressure. Irritation of the inflamed peritoneum in the pelvic appendix. Important in pelvic appendicitis. Kulenkampff sign Pain upon palpation of the right rectal wall during rectal exam. Irritation of inflamed pelvic structures. Confirms pelvic localization. Ten Horn sign Traction on the right spermatic cord elicits right lower quadrant pain. Tension is transmitted to the appendix. Seen in male patients. SURGERY 3'2025 Vol.89 Issue3 Online ISSN: 3033-151X, Print ISSN: 0450-2167 https://bgss.eu/ DOI: 10.5281/zenodo.17398511 20 Tender Points: • McBurney point – between the middle and outer third of the spinoumbilical line • Lanz point – between the outer and middle third of the interspinal line • Kummell point – below and to the right of the umbilicus (These are mainly relevant to chronic appendicitis) Auscultation: Of limited diagnostic value; bowel sounds are usually normal or slightly decreased, reflecting mild paralytic ileus. Clinical Presentation The typical clinical picture includes: • Pain starting peri-umbilically and migrating to the right iliac fossa, • Anorexia, • Nausea and/or vomiting, • Low-grade or moderate fever (up to 38–38.5°C), • Localized tenderness at McBurney’s point, • Muscular guarding, rebound tenderness (Blumberg sign), and positive specific tests (Rovsing, psoas, obturator) [3,7]. Meta-analyses show the most sensitive findings are pain migration (sensitivity 0.64–0.77), right iliac fossa tenderness (0.81), anorexia (0.79), and leukocytosis [3,4]. In children and adults over 65 years, the diagnosis is often delayed due to atypical presentations, resulting in higher rates of perforation and complications [3,5]. Screening and Clinical Scoring Systems Due to the absence of a single laboratory or imaging test with 100% diagnostic accuracy, several clinical algorithms have been developed to improve diagnostic precision. 1. Alvarado Score (1986) Consists of eight criteria—symptoms, physical findings, and laboratory parameters—with a maximum score of 10 points: • Migration of pain (1), • Anorexia (1), • Nausea/vomiting (1), • Right lower quadrant tenderness (2), • Rebound tenderness (1), • Elevated temperature (1), • Leukocytosis (2), • Neutrophilia (1). Interpretation: • <5 points: appendicitis unlikely • 5–6 points: observation and/or imaging recommended • ≥7 points: high likelihood of appendicitis – surgical management indicated [6] The system is widely used but less accurate in women and children. Modified Alvarado Score (MASS) Useful in settings where a differential white blood cell count (left shift) is not routinely assessed. The addition of an alternative clinical parameter in some studies demonstrated a sensitivity of 68% and a specificity of 58%. 3. Appendicitis Inflammatory Response (AIR) Score (2008) Incorporates C-reactive protein (CRP) levels and the degree of neutrophilia, which enhance diagnostic accuracy. It stratifies patients into three risk groups: • Low risk (0–4 points) • Intermediate risk (5–8 points) • High risk (9–12 points) The AIR Score demonstrates superior diagnostic performance compared to the Alvarado score, with an area under the curve (AUC) of 0.89–0.97 [2]. Pediatric Appendicitis Score (PAS) (2002) Designed specifically for children aged 4–15 years, this score includes eight clinical and laboratory parameters. A score of ≥6 points demonstrates a sensitivity of 100% and specificity of 92% [5]. The most predictive factors are pain on jumping or coughing, anorexia, and pyrexia. 5. RIPASA Score Particularly suitable and specific for Asian populations, the RIPASA score (an acronym derived from Raja Isteri Pengiran Anak Saleha Hospital, Brunei, 2010) was developed as an alternative to the Alvarado score, which showed lower sensitivity among Asian and Eastern European populations. Interpretation: • <7.0: Low probability of appendicitis • 7.5–11.5: High probability of acute appendicitis • >12.0: Very likely/complicated appendicitis 6. MZXBTCH Scoring System The MZXBTCH screening system was developed based on key clinical and imaging parameters associated with complicated appendicitis. The acronym “MZXBTCH” derives from the initials of its major predictive factors: M – Metastatic pain Z (CRP) – C-reactive protein X – X-ray imaging features (including appendiceal diameter) B – Body temperature T – Time from onset to visit C – Clinical signs of peritonitis H – Hematological markers (such as the neutrophil-to-lymphocyte ratio, NLR). These factors were identified through statistical analysis as significant predictors and incorporated into the scoring system to enhance usability and recall for clinicians. The MZXBTCH system was designed to predict complicated appendicitis based on independent risk factors established in a modeling cohort. Variables with statistical significance (P < 0.05) in univariate analysis were entered into a binary logistic regression model using backward stepwise elimination to identify independent predictors. Each predictor was assigned a weighted score proportional to its regression coefficient, reflecting its relative contribution to the risk of complicated appendicitis. The system integrates both clinical and imaging parameters, including preoperative CRP levels, NLR, signs of peritonitis, appendiceal diameter, and periappendiceal fat stranding. Using the R software, a nomogram was constructed to visualize the system and enable clinicians to calculate a cumulative risk score for each patient. СП. ХИРУРГИЯ • J. The MZXBTCH scoring system was subsequently validated in an independent cohort, where its diagnostic performance—including AUC, sensitivity, specificity, positive and negative predictive values, and overall accuracy—was compared with existing appendicitis scoring models. 7. Lintula Score Established: 2001 by Lintula et al. in Finland. Purpose: Early identification of acute appendicitis, particularly in outpatient settings. Advantage: Suitable for use in facilities without laboratory or imaging resources; relies primarily on clinical symptoms. Advantage: Utilizes common clinical and laboratory parameters that are easily available in routine practice. Score interpretation: • 0–2 points: Low probability of complicated appendicitis (<10%) • 3–4 points: Moderate probability (20–50%)
