Abs ac . Ch onic i on de iciency anemia (IDA) is commonly associa ed wi h s uc u al
and unc ional changes o he gas ic mucosa, including educed acidi y, mucosal a ophy, and
Helicobac e pylo i in ec ion. In his s udy, 48 IDA pa ien s we e examined and di ided in o wo
g oups based on gas ic acidi y: no mal (n = 26) and educed (n = 22). Pa ien s wi h low acidi y
had lowe se um i on (5.3 µmol/L s. 7.9 µmol/L) and e i in (7.8 ng/mL s. 11.6 ng/mL), mo e
equen mucosal a ophy (59% s. 19%), and highe p e alence o H. pylo i (77% s. 46%).
His ological analysis showed signi ican educ ion o pa ie al cells in he low-acidi y
g oup. These indings indica e ha unc ional and s uc u al gas ic mucosal changes con ibu e
o impai ed i on abso p ion in IDA, highligh ing he impo ance o gas ic assessmen o
op imizing ea men .

In oduc ion. Ch onic i on de iciency anemia (IDA) is one o he mos common
hema ological diso de s wo ldwide. In addi ion o sys emic mani es a ions, IDA is o en
associa ed wi h unc ional and s uc u al changes o he gas ic mucosa [1, 2]. Reduced gas ic
acidi y, mucosal a ophy, and Helicobac e pylo i in ec ion may impai i on abso p ion,
con ibu ing o pe sis en anemia [3, 4]. Unde s anding he unc ional s a e o he gas ic mucosa
in IDA pa ien s is essen ial o imp o ing diagnosis and op imizing ea men . The e o e, he aim
o his s udy was o e alua e he unc ional and s uc u al condi ion o he gas ic mucosa in
pa ien s wi h ch onic i on de iciency anemia and o in es iga e i s associa ion wi h i on
me abolism pa ame e s and H. pylo i in ec ion.
Ma e ials and me hods. A o al o 48 pa ien s wi h con i med ch onic IDA (hemoglobin
<120 g/L in women, <130 g/L in men; e i in <15 ng/mL) we e examined. Pa ien s we e di ided
in o wo g oups based on gas ic acidi y measu ed by pH-me y:
 G oup 1 (n = 26) – p ese ed gas ic acidi y (pH ≤ 3.5)
 G oup 2 (n = 22) – educed gas ic acidi y (pH > 3.5)
All pa ien s unde wen comple e blood coun , se um i on p o ile (i on, e i in, TIBC),
esophagogas oduodenoscopy (EGD) wi h biopsy, his ological e alua ion, and apid u ease es
o H. pylo i.
Resul s. Among 48 pa ien s, 26 (54.2%) had no mal gas ic acidi y, while 22 (45.8%)
showed educed acidi y. Pa ien s wi h low gas ic acidi y demons a ed wo se i on s a us: a e age
se um i on was 5.3 µmol/L, compa ed o 7.9 µmol/L in pa ien s wi h p ese ed acidi y. Simila ly,
a e age e i in was 7.8 ng/mL in he low-acidi y g oup e sus 11.6 ng/mL in he no mal-acidi y
g oup. Endoscopic examina ion e ealed ch onic gas i is in 42 pa ien s (87.5%). Gas ic mucosal
a ophy was mo e equen in pa ien s wi h educed acidi y (59%) han in hose wi h no mal
acidi y (19%).

pa ien s wi h hypoacidi y, while only 23% o pa ien s wi h no mal acidi y showed mild changes.
H. pylo i in ec ion was de ec ed in 29 pa ien s (60.4%), wi h highe p e alence in he low-acidi y
g oup (77%) compa ed o he no mal-acidi y g oup (46%).
O e all, hese indings indica e ha pa ien s wi h ch onic IDA o en ha e s uc u al and
unc ional gas ic mucosal changes, pa icula ly hose wi h educed acidi y. Mucosal a ophy,
pa ie al cell educ ion, and H. pylo i in ec ion a e associa ed wi h poo e i on abso p ion and
lowe i on s o es [5].
Conclusion. Ch onic i on de iciency anemia is equen ly accompanied by signi ican
unc ional and s uc u al abno mali ies o he gas ic mucosa. Assessmen o gas ic sec e ion and
mucosal in eg i y is essen ial o op imizing diagnosis and he apy, imp o ing i on abso p ion, and
enhancing ea men e ec i eness in IDA pa ien s.
