Com! a! introdução! de! novas! tecnologias! de! diagnóstico! e! de! plano! de! tratamento,! o! conceito!de! cirurgia! guiada! tem! vindo! a! ser! aceite,! passandoKse!a!considerar!primeiro!a!prótese!e!posteriormente!a!cirurgia.! Desenvolvimento:'Esta! técnica! está! indicada! em! várias! situações! e! possui!vantagens!e!desvantagens,!que!devem!ser!tidas!em!conta!para! que!as!complicações!e!os!erros!sejam!evitados.!A!cirurgia!guiada!pode! ser! realizada! de! maneira! estática! ou! dinâmica,! e! dentro! da! estática! podem! ser! utilizados! três! tipos! de! guias! cirúrgicas:! dentoKsuportadas,! mucoKsuportadas! e! ósseoKsuportadas.! Abstract' ' Introduction:!The!goal!of!dental!implants!therapy!is!the!predictable!and! accurate!restoration!of!the!patient’s!dentition.!With!the!introduction!of! new! technologies! of! diagnosis! and! treatment! plan,! the! concept! of! guided! surgery! has! been! progressively! accepted,! considering! first!the! prosthesis!and!subsequently!the!surgery.! Objective:!The!aim!of!this!article!is!to!deepen!the!knowledge!in!an!area! of!professional!and!personal!interest!and!to!be!aware!of!an!additional! technique! for! implant!placement,! in! addition! to! the! so! called! conventional!technique,! allowing! the! rehabilitation! of! patients! with! anatomical! and! morphological! factors! which! make!implant! placement! more!difficult.' Material' and' Methods:!Electronic! research! on! the! PubMed! database! using!combinations!of!the!following!terms:' “dental!implants”,!“guided! surgery”,! “computerKguided! surgery”,! “computerKaided! surgery”,! “oral! rehabilitation”,! “immediate! loading”,! “delayed! loading”.! The! research! was!limited!to!case!report!and!review!articles!published!in!English!in!the! last!10!years,!in!humans.! Bibliographic'Review:!This!technique!can!be!used!in!various!situations! and! it! has! advantages! and! disadvantages! which!must! be! taken! into! account! so! that!complications! and! errors! are! avoided.! The!guided! surgery! can! be! performed! statically! or! dynamically,! and! in! the! static! approach!three!types!of!surgical!guides!can!be!used:!toothKsupported,! mucosalKsupported! and! boneKsupported.! When!all! preKoperative! planning!stages!and!the!surgical!procedure!are!concluded!the!patient!is! rehabilitated!with!an!immediate!prosthesis.! Introdução' ' !No!início!de!1980,!a!colocação!de!implantes!era!realizada!apenas! com!base!no!osso!residual!disponível.!Os!implantes!posicionados!desta! maneira! frequentemente! emergiam! numa! posição! bucal! ou! lingual,! originando!problemas! estéticos! difíceis!ou! mesmo! impossíveis! de! resolver.!Devido!aos!problemas!associados!e!a!compromissos!funcionais! da!prótese!final,!novos!conceitos!foram!desenvolvidos!e!novos!métodos! produzidos,!nos!quais!se!considera!primeiro!a!prótese!e!posteriormente! a! cirurgia.! Introduzindo! o! conceito! de! “implantologia! guiada! pela! prótese”,!não!só!o!osso!é!considerado!como!também!se!consideram!os! dentes!a!serem!substituídos![1].!! O! objetivo! da! terapia! com! implantes! dentários! é! a! restauração! previsível! e! precisa! da! dentição! do! paciente.! A! recente! introdução! de! novas! tecnologias! de! diagnóstico! e! de! plano! de! tratamento! a! 3! dimensões!proporcionaram!um!ambiente!para!a!abordagem!em!equipa! do!planeamento!e!colocação!de!implantes!dentários,!de!acordo!com!um! plano!de!tratamento!impulsionado!pela!restauração!protética![2].!Isto!é,! na! cirurgia! guiada! deve! ser! feito! um! planeamento! inverso,! em! que! primeiro! é! realizado! um! plano! restaurador!protético,! criando! um! diagnóstico!do!alinhamento!dentário!em!que!esteja!indicada!a!anatomia! dentária!e!a!posição!dos!dentes!a!serem!substituídos![3].!! Na! reabilitação! de! perda! dentária! com! implantes,! um! planeamento! préKcirúrgico!minucioso! é! um! requisito!importante! para! um! resultado! restaurador! de! sucesso![4]!e! inclui! considerações! anatómicas,!bem!como!protéticas!para!a!posição!precisa!dos!implantes! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 3! [5].! Usando! protocolos! cirúrgicos! tradicionais/convencionais,! o! planeamento!préKcirúrgico!inclui!uma!avaliação!radiográfica!do!volume! ósseo!acessível!e!das!estruturas!anatómicas,!o!que!na!maior!parte!dos! casos! é! determinado! por! radiografias! panorâmicas! e! periapicais![4]! combinadas!com!a!inspeção!visual!e!palpação!clínica;!contudo,!podem! ser! insuficientes! para! obter! o! melhor! planeamento! préKcirúrgico!em! casos! complexos! ou! comprometidos.! Nesses! casos,! o! uso! de! técnicas! imagiológicas!de!3!dimensões!podem!fornecer!uma!dimensão!extra!às! radiografias! préKcirúrgicas!rotineiras! disponíveis,! providenciando! informação!mais!detalhada!em!relação!ao!volume!e!qualidade!óssea!ou! restrições!anatómicas!![5].!Na!técnica!cirúrgica!tradicional/convencional,! o! posicionamento! do! implante! é! avaliado! pelo! julgamento! combinado! do!volume! ósseo! nas! radiografias! e! pela! inspeção!visual! da! crista! alveolar!na!cavidade!oral!e!nos!modelos!de!estudo![4].!A!perfuração!na! crista!alveolar!é,!inevitavelmente,!um!procedimento!“cego”,!já!que!parte! da! perfuração! óssea,! defeitos! anatómicos! e! estruturas! vitais! na! vizinhança! da! perfuração!não! são!visíveis.! Lesões! iatrogénicas! podem! ocorrer!e!são!eventos!desagradáveis,!tanto!para!o!médico!dentista!como! para!o!paciente![6].! !Usando! CT/CBCT! scanners,! a! visualização! da! altura! e! largura! do! osso!disponível!para!a!colocação!do!implante,!a!espessura!dos!tecidos! moles,!a!proximidade!e!a!anatomia!das!raízes!dos!dentes!adjacentes,!a! localização!exata!dos!seios!maxilares,!seio!do!septo!e!outras!estruturas! vitais! pertinentes,! como! o! canal! mandibular,! o! buraco! mentoneano! e! canal! incisivo! são! possíveis.! Uma! vez! importadas! as! imagens! para! os! programas! de! software!apropriados! (SimPlant,! NobelClinician,! entre! outros),! o! médico! dentista!pode,! então,! planear! virtualmente! a! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 4! colocação!de!implantes!de!acordo!com!a!anatomia!e!o!caso!do!paciente! em!questão.!O!tipo!e!tamanho!do!implante!planeado,!a!sua!posição!no! osso,! a! sua! relação! com! a! restauração! protética! planeada! e! com! os! dentes!e/ou!implantes!adjacentes,!e!a!sua!proximidade!com!estruturas! vitais! podem! ser! determinadas! antes! de! se! realizar! a! cirurgia! no! paciente.!As!guias!cirúrgicas!podem!então!ser!fabricadas!de!acordo!com! o! plano! de! tratamento! virtual.! Essas! guias! são! usadas! pelo! médico! dentista!para! colocar! os! implantes! planeados! na! mesma! posição! do! plano! de! tratamento! virtual,! permitindo! uma! precisão! e! uma! previsibilidade!na!colocação!do!implante!e!uma!redução!da!morbilidade! do!paciente![2].! !Hoje! em! dia,! há! três! modelos!para!aplicar! esta! técnica:! cirurgia! guiada! usando! guias! de! perfuração! processadas! por! prototipagem! estereolitográfica!rápida,! computer6milled% templates!e! sistemas! de! navegação!computorizados.!! Computer6milled% templates!são! fabricados! pela! perfuração! na! posição!final!dos!implantes!no!próprio!modelo!digitalizado,!usando!uma! máquina!de!perfuração![5].!! Nos! sistemas!de! navegação! computorizados,! os! implantes! são! guiados!com!assistência!ótica!e! real6time% tracking!da!broca! de!acordo! com!a!trajetória!préKcirúrgica!planeada![4,!5].!! Em!todos!os!casos,!a!elevação!de!retalho!mucoso!para!a!inspeção! da!anatomia!óssea!é!desnecessária.!! !O!objetivo!primário!desta!técnica!é!melhorar!a!precisão!protética,! cirúrgica! e! de! diagnóstico,! mas! hoje! em! dia!temKse! centrado! principalmente! numa! direção! de! utilização! rápida! e! simplificada.! Este! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 11! normal!é!acelerada![2,!3,!11],!os!resultados!estéticos!são!melhores![11]!e! é!preservada!a!arquitetura!dos!tecidos!moles!e!duros![12].!A!inserção! flapless!de!implantes!tem! taxas!de!sucesso!comparáveis!às!da!técnica! convencional! com! retalho! (non6flapless),! enquanto! minimiza! as! complicações!da!elevação!de!retalho!de!tecido!mole!como!a!infeção,!a! deiscência!e!a!necrose!de!tecidos!moles!e!duros![2,!3].!! A! cirurgia! de! implantes! guiada!envolve! um! planeamento! do! tratamento! inverso,! isto! é,! a! confeção!de! uma! restauração! protética! antes!da!inserção!dos!implantes.!O!contorno!ideal!e!a!posição!do!arco!da! restauração!protética!são!planeados!primeiro,!seguidos!do!planeamento! virtual! da! posição! do! implante,! de! acordo! com! a! anatomia! óssea! e! o! cenário! clínico.! As! guias! cirúrgicas!estereolitográficas!são! depois! fabricadas! a! partir! do! plano! de! tratamento! e! aquando! da! cirurgia!são! usadas! para! a! colocação! de! implantes! de! modo! flapless,! removendo! apenas!uma!porção!de!tecido!no!local!do!implante![2,!3].! A! orientação! cirúrgica! da! profundidade! de! perfuração!e! a! angulação,! em! combinação! com! uma! técnica! flapless,! minimizam! a! potencial! lesão! de! estruturas! anatómicas! subjacentes! durante! a! preparação!da!osteotomia!para!a!colocação!do!implante![2].!Os!pilares! são! colocados! imediatamente! e! as! restaurações!protéticas!provisórias! inseridas.!Essas!restaurações!protéticas!permitem!ao!médico!dentista!e! ao! técnico! de! prótese!recolher! informação! quanto! à! estética! e! ao! contorno!gengival!através!da!observação!da!resposta!dos!tecidos!moles! às! restaurações! provisórias,! permitindo! assim!a! confecção! de! uma! restauração! definitiva! o! mais! adaptada! possível! [2,! 3].! A!redução! do! tempo! de!tratamento! é! a! maior! vantagem!da! inserção! imediata! da! restauração!definitiva/provisória!após!a!colocação!dos!implantes![2].! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 12! Posição!do!implante!é!crítica!para!a!restauração!–!colocação!de! únicos!ou!múltiplos!implantes!na!zona!estética.!A!espessura!dos!tecidos! moles! bucais! e! da! crista! e! das! placas! corticais! bucais! e! palatinas,! a! dimensão!bucoKlingual!da!crista,!a!proximidade!com!dentes!adjacentes,! a! relação! raízKimplante,! o! suporte! e! o! contorno! gengival! e! papilar,! a! exposição! gengival,! a! linha! do! sorriso! e! a! emergência! e! angulação! do! implante! são! algumas! das! muitas! considerações! complexas.! Uma! importante! consideração! protética! é! o! conhecimento! da! posição! apropriada! do! implante! baseada! no! tipo! de! restauração!protética! planeada,! uma! vez! que! a! sua! posição! pode! ser! crítica! para! o! sucesso! estético![2,!3],!fonético![4]!e!funcional!da!restauração,!já!que!pequenas! variações!na!sua!posição!podem!comprometer!a!restauração![2,!3].! Pacientes! com! debilidades! físicas,! médicas! ou! psicológicas! –! pacientes!com!cancro!na!zona!da!cabeça!e!do!pescoço!são!muitas!vezes! tratados! com! radioterapia! pré! e/ou! pósKoperatória,! o! que! pode! potenciar!alterações!na!vascularização!dos!tecidos!moles!e!ósseos!e!na! capacidade!de!cicatrização.!A!colocação!de!implantes!com!o!mínimo!de! elevação!de!retalho!e!de!trauma!dos!tecidos!moles!e!duros!é!indicada! para! limitar! a! probabilidade! destes! pacientes! virem!a! desenvolver! osteoradionecrose! na! maxila! e/ou! mandíbula.! Hemorragia,!edema! e! alterações!da!vascularização!óssea!e!dos!tecidos!moles!são!minimizadas! usando!estas!tecnologias!de!planeamento!e!diagnóstico![2,!3].!! Pacientes!com!discrasias!sanguíneas,!problemas!de!coagulação!e! doenças!cardiovasculares!podem!precisar!de!medicação!específica!antes! da! intervenção! e! deve!também! diminuirKse!o! risco! de!hemorragia! minimizando!o! trauma! cirúrgico.! A! avaliação! e! planeamento! em!3D! permite! a! colocação! dos!implantes! de! modo! flapless!com!exatidão! e! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 13! precisão.! Pacientes! com! mobilidade! reduzida,! stressados,! ansiosos! e! com!fobias,!bem!como!com!problemas!ortopédicos!e!da!coluna,!muitas! vezes!necessitam!de!um!planeamento!e!preparação!extensivos!antes!do! tratamento,! prevenindo! assim!que! estes! sejam! submetidos! a! procedimentos! que! requerem! longos! períodos! de! tempo! na! cadeira! dentária!sem!comprometer!a!qualidade!do!tratamento![2,!3].! Usando!tecnologias! 3D,! a! maior! parte! do! planeamento! e! dos! problemas!anatómicos!encontrados!durante!a!cirurgia!são!visualizados! antes! do! paciente! estar! sentado! na! cadeira! dentária.! Usando! guias! cirúrgicas,! os! implantes! são! colocados! de! maneira! rápida! e! previsível,! minimizando!o!stress,!a!dor,!o!tempo!de!cadeira!do!paciente![2,!3],!o! edema! e! o! desconforto! pósKcirúrgico! [4],! com! o! tempo! cirúrgico! reduzido!a!um!mínimo![2,!3].!Sendo!assim,!uma!técnica!minimamente! invasiva! é! vantajosa! quando! estamos! perante! este! tipo! de! pacientes! vulneráveis![4],!que!seriam!excluídos!em!procedimentos!convencionais! de!implantes![5].! Vantagens'e'Desvantagens' Uma!das!vantagens!desta!técnica!cirúrgica!prendeKse!com!o!!facto! de! que! os! procedimentos! de! aumento!de! volume!ósseo! podem! ser! evitados!ou!reduzidos!através!da!otimização!da!posição!do!implante!no! osso!presente![4,!13].!De!realçar!também!o!benefício!assumido!de!um! diagnóstico! préKoperatório! minucioso! e! de! um! procedimento! de! implantação! mais! previsível! em! concordância! com! as! estruturas! anatómicas!e!os!aspetos!protéticos,!bem!como!a!possibilidade!do!préK fabrico! de! restaurações!protéticas!e! de! carga! imediata! e,! em! alguns! casos,!o!recurso!a!procedimentos!flapless![13].! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 14! Mas! também! há! desvantagens.! O! CT! scan!expõe! o! paciente! a! doses! de! radiação! mais! elevadas! do! que! as! técnicas! convencionais! de! radiografia,!produz!artefactos!dispersos!das!restaurações!metálicas,!é!de! custo!elevado!para!os!pacientes,!a!transferência!de!informação!para!um! modelo!cirúrgico! é! difícil! e! necessita! de! programas! de! software! adicionais!de!processamento!de!imagem,!a!interpretação!das!imagens!é! difícil,!sendo!necessário!treino!prévio,!e!há!a!possibilidade!de!o!paciente! se! mover! durante! a! exposição! [14].! Uma! outra! desvantagem! desta! técnica! cirúrgica,! comparando! com! uma! técnica! com! retalho! mucoperiósseo! (open6flap),! é! que! esta! possui! uma! abordagem! “cega”! devido!à!não!visualização!do!osso!subjacente![12].! Complicações' O!conceito!de!cirurgia! de! implantes! guiada!por! computador!em! combinação! com! carga! imediata! é! “comercializado”! como! sendo! fácil,! seguro! e! previsível.! Contudo,! várias! complicações! ou! eventos! inesperados!durante!a!colocação!guiada!dos!implantes!e!da!restauração! protética!vão!sendo!reportados.!A!complicação!cirúrgica!mais!comum!foi! a!fratura!da!guia!cirúrgica!e!a!complicação!protética!mais!comum!foi!o! não!assentamento!correto!da!prótese.!Quando!se!usa!a!técnica!cirúrgica! convencional!de!implantes,!este!tipo!de!eventos!não!se!aplicam!ou!não! são!referidos!como!complicações![4].!! Contudo,! parece! óbvio! que! a! técnica! cirúrgica! guiada,! especialmente! em! combinação! com! a! carga! imediata,! não! pode! ser! considerada! como! mais! fácil! do! que! a! técnica! convencional.! As! complicações!mais!referidas!depois!da!colocação!guiada!dos!implantes! são!a!falha!dos!mesmos!e!da!referida!prótese.!A!taxa!de!insucesso!dos! !Cirurgia!Guiada!na!Colocação!de!Implantes! ! ! ! 15! implantes! reportada!é!de!3%!comparada! com!a!cirurgia!convencional.! Em! alguns!casos,!também!ocorrem!complicações!préKcirúrgicas,!como!foi!o! caso!em!9,1%!de!428!pacientes!tratados!(Tabela!I)![13].!! ! ! ! ! ! ! ! ! ! ! extensive occlusal adjustments (4.3%) are described. Late prosthetic complications, reported in three studies, occurred in 13 (12%) cases and may be associated with the prosthesis material or improper seating: Fractures of the prostheses (2.8%), of the veneering material (1.9%) and screw loosening (2.8%) are described. The tolerance and effect of specially designed abutments to compensate for a certain amount of deviation between implant and prosthesis position seems to be limited. A higher accuracy and reproducibility of implant position as well as optimization of the prosthetic components and fabrication techniques might allow immediate final restorations in the future. In comparison with a recent review on complications related to not computerassisted implant rehabilitation (Gervais & Wilson 2007), the incidence of technical complications is higher than in studies included in the present review. The occurrence of technical complications with fixed, implant-supported prostheses includes fractures of the prostheses in 3%, acrylic veneer fracture in 22%, ceramic veneer fracture in 14% and fractures or loosening of abutment or prostheses screws in 17% of the prostheses. Because of the different prosthesis designs, loading protocols, observation periods and since a generally accepted definition of a prosthetic or surgical complication is inexistent and therefore unforeseen events may not always be reported, the data must be interpreted and with caution. After a follow-up of 12–60 months an implant survival rate of 91–100% was reported in a total of six studies with 79 patients and 587 implants. Keeping in mind that in four out of six studies implants were inserted in fully edentulous patients and immediately loaded the implant failure rates are similar to conventional procedures (Pjetursson et al. 2004; Esposito et al. 2007). However, due to the relatively short observation period and low variety of systems used, further investigations are necessary to confirm the high long-term implant survival. Conclusions Based on the data analysis of this systematic review it is concluded that various systems for computer-guided templatebased implant treatment are available. Different types of software, template production and template stabilization as well as variations of the surgical and prosthetic protocol are reported. Meta-analysis of in vitro,cadaverandclinicalstudiesregarding accuracy revealed mean horizontal deviations of 1.1 –1.6 mm, but also considerably higher maximum deviations. The survival rate of implants placed with computerguided technology is comparable to conventionally placed implants ranging from 91% to 100% after an observation time of 12–60 months. Early surgical complications were observed in 9.1%, early prosthetic complications in 18.8% and late prosthetic complications in 12% of the patients. However, limited data and relatively short observation periods are available in literature. Further research should involve clinical studies with long-term follow-up and strive for an improvement of the systems and procedures regarding accuracy, predictability and reproducibility of implant placement as well as surgical and prosthetic outcomes. One can compare this threedimensional condition with images from three cameras that are following the implant, and where the clinician can look at the image at any time on one, two or three cameras, depending on the need. The use of (cone beam) computed tomography involves the use of a high dose of radiation, however, this should be considered as acceptable in view of the added clinical value provided by the images (29). Moreover, even with the most optimal preoperative planning software, transfer to the surgical field still needs to achieve clinical and medico-legally acceptable accuracy (55). Several options are available for such transfer: computer-guided (static) surgery; or computer-navigated (dynamic) surgery (30). For computer-guided surgery a static surgical guide is used that transfers the virtual implant position from computed tomography data to the surgical site. These guides are produced by computer-aided design/computer-assisted manufacture technology, such as stereolithography, or manually in a dental laboratory (using mechanical positioning devices or drilling machines) (52, 59). With computer-navigated surgery the position of the instruments in the surgical area is constantly displayed on a screen with a three-dimensional image of the patient. In this way, the system allows real-time transfer of the preoperative planning and visual feedback on the screen (4, 48, 71). A workflow of the different systems is presented in Fig. 1. Static surgical guides Stereolithography As mentioned before, besides the bone volume, the ideal tooth position is visualized via a scan prosthesis, so that the implants can be positioned taking both the anatomic and prosthetic aspects into account. As a standard resin prosthesis has a density similar to that of the surrounding soft tissues, it is impossible to segment it easily from the computed tomography images. Therefore, a special scan prosthesis has to be prepared. This can be performed in several ways. The first option is to prepare a copy of the prosthesis in radiopaque resin (Fig. 2A). Only one scan has to be made with the patient wearing the prosthesis in the mouth. A second method was developed in the mid-1990s by a research team at the University of Leuven. They proposed a double-scan procedure (the patient with the scan prosthesis in the mouth; and the prosthesis alone) followed by integration of the scan prosthesis or radiological template, planned by the dentist, within the craniofacial model (61–63). Therefore, the scan prosthesis contains small gutta percha spheres (diameter !1 mm) (Fig. 2B). The craniofacial images show the gutta percha markers with respect to the bone, without visualizing the prosthesis itself. Metal tubes, used as drill sleeves, are inserted through the holes, previously created by the drilling machine, in the surgical guide. Different guides with different diameters are prepared and have to be placed consecutively or one can use drill keys. In partially edentulous patients, the guide is supported by residual teeth. In full maxillary edentulous patients the guide is supported by the mucosa, especially the hard palate area (Fig 11G,H). The protocol can be summarized as follows: Step 1. Production of the scan prosthesis with radiopaque teeth + X-cube. Step 2. Computed tomography scan with the scan prosthesis in the mouth of the patient. Step 3. Planning using the software. Step 4. Drilling the implant positions in the scan prosthesis with the numerically controlled drilling machine (fully automatic). The scan prosthesis becomes the surgical guide. Step 5. Surgery. The guide is then easily replaced in the mouth of the patient, in the same position as during computed tomography examination. Navigation (dynamic systems) Surgical navigation systems are able to track a surgical tool relative to the patient, and to dynamically display the position of the surgical tool within the patient’s presurgical computed tomography scan, updated in real time (13, 14, 21, 83). Thus, navigation systems allow for: localization of surgical targets and critical anatomical structures; orientation of a surgical tool within the patient’s anatomy; and navigation of a surgical tool along a predefined surgical plan. Tracking technology Navigation systems for oral and craniomaxillofacial surgery are based on optical tracking technology (13, 83) (Fig. 12A). The technology can be compared with the guidance of cars using the global positioning system. Similarly to the car with the global positioning system device that is tracked by a satellite and guided along a predefined route on a map, the surgical drill with light emitting diodes or passive reflecting tracking elements is tracked by a stereoscopic optical camera and guided along a predefined implant plan on the computed tomography data (Fig. 12B). The accuracy of optical tracking currently lies within a range of 0.1–0.4 mm (31). In order to track the position of the moveable head of the patient, a dynamic reference frame is mounted on the patient (65, 66, 69). The dynamic reference frame can be invasively fixed to the bone or noninvasively mounted on a denturefixed template (5, 6, 84, 85) (Fig. 12C). To calculate the angular deviation, the central axis line coordinates were used to determine 2 vectors, and the vectors were used to calculate the deviation with the following formula derived by the technician: where the numerator is the dot product of the 2 vector quantities and the denominator is the product of their magnitudes. A computer program developed by the technician for the purpose of the study computed the vectors and the angle. Mean deviations for each type of surgical guide (50 implants for each type of guide) and the overall mean deviation were determined. An a priori power analysis based on pilot data indicated that for angular deviation a sample size of n=50 per group would provide 85% power to detect a mean difference of 1.0 degree, assuming that the common standard deviation is 1.42 (observed range 0.22-3.5 degrees) using an independent samples t test with the 2-sided alpha level set at .016. For planning purposes, alpha was set at .016 to apply a Bonferroni adjustment since 3 pairwise contrasts were planned (α=.05/3=.016). For linear displacement, a sample size of n=50 per group would have 99% power to detect a mean difference of 0.50 mm, assuming that the common standard deviation is 0.34 mm (SD measured at implant hex=0.34 mm; SD measured at implant apex=0.32 mm) using an independent samples t-test with 2-sided alpha set at .016. The dependent variables of interest were the angular and linear deviations relative to the planned implant locations. Separate analyses were performed for angular displacement and linear displacement measured at implant hex and base. The manipulated independent variable was the anatomic base from which simulated treatment planning was done (3 levels: bone, mucosa, tooth). Statistical analyses were performed with a software program (SPSS, Inc, Chicago, Ill). A linear mixed models approach was used, and individual implant location with 5 levels represented a repeated measures factor. All reported P values were adjusted by the DunnSidak method to control the Type I error rate across multiple pairwise comparisons. RESULTS A total of 150 implants were placed without complication. Fifty implants were placed into acrylic resin mandibles with canines and first molars with tooth-supported guides; 50 implants were placed into completely edentulous acrylic resin mandibles with bone-supported guides; and 50 implants were placed into completely edentulous acrylic resin mandibles with a soft tissue material simulating mucosa with mucosa-supported guides. The mean angular deviation of the long axis between the planned and placed implants was 2.2 ±1.2 degrees; the mean deviations in distance between the planned and placed implants were 1.18 ±0.42 mm at the implant neck and 1.44 ±0.67 mm at the implant apex for all 150 implants. After the superimposition procedure, the angular deviation of the placed implants with the tooth-supported guide was 2.26 ±1.30 degrees. The angular deviation with the bonesupported guide was 2.17 ±1.02 degrees, and the angular deviation with the mucosa-supported guide was 2.29 ±1.28 degrees. The mean deviations in distance between the planned and placed implants at the neck and apex were 1.00 ±0.33 mm and 1.15 ±0.42 mm for the tooth-supported guides, 1.08 ±0.33 mm and 1.53 ±0.90 mm for the bone-supported guides, and 1.47 ±0.43 mm and 1.65 ±0.48 mm for the mucosa-supported SLA surgical guides. After the matching procedure and statistical analysis, there was no statistically significant difference among any of the 3 guides for angular deviation (Fig. 9). To calculate the angular deviation, the central axis line coordinates were used to determine 2 vectors, and the vectors were used to calculate the deviation with the following formula derived by the technician: where the numerator is the dot product of the 2 vector quantities and the denominator is the product of their magnitudes. A computer program developed by the technician for the purpose of the study computed the vectors and the angle. Mean deviations for each type of surgical guide (50 implants for each type of guide) and the overall mean deviation were determined. An a priori power analysis based on pilot data indicated that for angular deviation a sample size of n=50 per group would provide 85% power to detect a mean difference of 1.0 degree, assuming that the common standard deviation is 1.42 (observed range 0.22-3.5 degrees) using an independent samples t test with the 2-sided alpha level set at .016. For planning purposes, alpha was set at .016 to apply a Bonferroni adjustment since 3 pairwise contrasts were planned (α=.05/3=.016). For linear displacement, a sample size of n=50 per group would have 99% power to detect a mean difference of 0.50 mm, assuming that the common standard deviation is 0.34 mm (SD measured at implant hex=0.34 mm; SD measured at implant apex=0.32 mm) using an independent samples t-test with 2-sided alpha set at .016. The dependent variables of interest were the angular and linear deviations relative to the planned implant locations. Separate analyses were performed for angular displacement and linear displacement measured at implant hex and base. The manipulated independent variable was the anatomic base from which simulated treatment planning was done (3 levels: bone, mucosa, tooth). Statistical analyses were performed with a software program (SPSS, Inc, Chicago, Ill). A linear mixed models approach was used, and individual implant location with 5 levels represented a repeated measures factor. All reported P values were adjusted by the DunnSidak method to control the Type I error rate across multiple pairwise comparisons. RESULTS A total of 150 implants were placed without complication. Fifty implants were placed into acrylic resin mandibles with canines and first molars with tooth-supported guides; 50 implants were placed into completely edentulous acrylic resin mandibles with bone-supported guides; and 50 implants were placed into completely edentulous acrylic resin mandibles with a soft tissue material simulating mucosa with mucosa-supported guides. The mean angular deviation of the long axis between the planned and placed implants was 2.2 ±1.2 degrees; the mean deviations in distance between the planned and placed implants were 1.18 ±0.42 mm at the implant neck and 1.44 ±0.67 mm at the implant apex for all 150 implants. After the superimposition procedure, the angular deviation of the placed implants with the tooth-supported guide was 2.26 ±1.30 degrees. The angular deviation with the bonesupported guide was 2.17 ±1.02 degrees, and the angular deviation with the mucosa-supported guide was 2.29 ±1.28 degrees. The mean deviations in distance between the planned and placed implants at the neck and apex were 1.00 ±0.33 mm and 1.15 ±0.42 mm for the tooth-supported guides, 1.08 ±0.33 mm and 1.53 ±0.90 mm for the bone-supported guides, and 1.47 ±0.43 mm and 1.65 ±0.48 mm for the mucosa-supported SLA surgical guides. After the matching procedure and statistical analysis, there was no statistically significant difference among any of the 3 guides for angular deviation (Fig. 9). However, the guides were not fabricated with stereolithography but were laboratory-made. Based on the results, the authors concluded that computerassisted planning provides good accuracy and that deviations were most likely related to reproducibility errors; a problem that could be decreased with the use of SLA guides. Ruppin et al23 compared an optical tracking splint system to a stereolithographic splint system. The authors did not find a statistical difference between the different methods; however, they did report that the accuracy achieved corresponded with the spatial resolution of the CT used. Most of the published literature to date on this topic has used traditional CT scans. Giacomo et al24 placed 21 implants in 4 patients by using traditional CT images. Traditional CT images were used to virtually plan the implants, and SLA guides were fabricated. CT scans made after surgery allowed the authors to determine the accuracy of implant placement. The introduction of digital systems into the diagnostic routine, and their application using three dimensional data sets of the bone topography, have made it possible to reconcile the two central aspects of planning an ideal prosthetic solution with the given anatomic situation prior to implant surgery. Threedimensional data from a computerized tomography (CT) scan or from digital volume tomography (DVT) scan in DICOM format (Digital Imaging and Communications in Medicine) can be represented, analyzed, and used for the subsequent fabrication of a surgical template with the help of special software applications. Such surgical guides are produced in a stereolithographic process using digital data (NOBEL GUIDE, Nobel Biocare) and integrate the appropriate sleeves for the implant positioning. This offers considerable advantages for both the patient and the clinician. The available bone can be used optimally and bone grafting can be avoided, or planned in advance in the best possible way if a bone graft is necessary. By combining the digital information of the bone with the planned restorative solution, the implant position can be adjusted to the best possible angulations, position, and depth. In addition, it is possible to accurately prepare a provisional or permanent restoration on the cast prior to the surgical procedure and to use it as an immediate provisionalization. This results in a predictable surgical outcome, and subsequently, a restorative treatment outcome (4). The aim of this clinical report is to document the protocol of sophisticated pre-operative diagnostic implant planning and a flapless surgical approach with immediate loading in the rehabilitation of partial edentulies (5-6). Diagnostic, prosthetic and surgical protocol Before any treatment all patients are clinical examinated. In addition to general health requirements for conventional implant treatments, patients have to be able to open the mouth at least 5 cm (prerequisite to accomodate the surgical drills). However in case of limited mouth opening is sufficient give to the patient, for three mounth before surgery, a neuro-muscolar decontizioning therapy and plan the implants maximum until the first molar (7). After this important valutation we can start the computer guided implant protocol that can be divided in six stages: radiographic guide preparation; CT scan; computer planning; provisional restoration fabrication; surgical procedure; and provisional restoration adjustment. The preparation of the radiographic guide for partial rehabilitation follows the same rules used for full arch rehabilitation with some little differents. It starts with the articolation of the stone model using a silicone bite registration index, and making an ideal wax-up of the missing teeth. After that the laboratory technician realize the radiographic guide covering with resin material (with a thickness between 2 and 3 mm) buccal, occlusal and palatal sides of the remaining teeth (Fig. 1). A full palatal recovering is important to achieve an optimal retention of both radiographic and surgical template (that will be realized on the basis of the radiographic guide). In addition we suggest to make some inspection windows through the occlusal surface of the existing teeth so that they are visible Figure 1 Radiographic guide. This ideal diagnostic wax-up is then incorporated, by a dental laboratory, into an acrylic prosthesis, referred to as a radiographic guide, scan prosthesis or scan appliance. Depending on the system to be used, this scan prosthesis can be a partial or full denture (Figs. 1–3.) Most systems, other than NobelClinician, require that the planned restorations contain a20%to30%bariumsulfatemixtureintheacrylictoallowforradiopacityoftheplannedrestorationsinthe CT/CBCT images. NobelClinician uses a double-scan technique with a hard acrylic scan prosthesis and gutta percha markers as reference points, with no barium sulfate. According to the individual system protocols, the CT/CBCT scan is then taken with the patient wearing the scan prosthesis. The CT scan DICOM (Digital Imaging and Communication in Medicine) images are then imported into the various proprietary software programs (SimPlant, NobelClinician, EasyGuide, and so forth). The software programs are then used to virtually place implants into their ideal position related to the planned restoration and the underlying bony anatomy (Figs. 4–6.)The digital treatment plan is then downloaded to the manufacturer for fabrication of a surgical guide (Figs. 7–9). The surgical guide is used, with implant-specific drilling instrumentation, to precisely place the implants in the positions, depths, and angulations as planned virtually. Many CT-guided implant planning technologies require radiopaque fiducial reference markers to be placed in the scan prosthesis that the patient wears during the CT/CBCT scan. The software uses these reference markers to virtually position the scan appliance, and with it the parameters of the planned restoration(s), to the patient’s jaw. The accurate assessment of these geometric markers can be difficult for some CBCT scanners and has the potential to add error into a precise planning system, ultimately leading to inaccurate fitting of surgical guides and error in implant placement. It is advisable to make every effort to investigate which CBCT scanners have high levels of accuracy or to use medical CT scanners when using these technologies [32]. Fig. 1. (Fig. 3). As the markers are visible in both sets of scans, they can be transformed and realigned to fuse the prosthesis within the maxillofacial structures. Besides an adequate bone model, derived from scanning the patient with the denture in situ, the second scan allows optimal visualization of the prosthesis. Therefore, both models can be presented separately, allowing planning on the bone (Fig. 4A) and/or the prosthetic model. Moreover, by accurate fusion, whilst maintaining excellent image quality, the planning can be carried out and controlled toward the integrated model (Fig. 4B) (27, 58). Regardless of the method used, correct positioning of the scan prosthesis is very important. Therefore, an index is strongly recommended to position and stabilize the template in the mouth of the patient during the scanning process (Fig. 5). An optimal fit of the scan prosthesis with the patient’s soft tissue is crucial. One should determine whether air is visible between the scan prosthesis and the soft tissue. This is especially important for mucosa-supported guides, in which the basis of the future surgical guide will be the same as the basis of the scan prosthesis. The DICOM images are imported in a software program, fusion of the scan prosthesis via the markers is accomplished and the ideal surgical site and optimal implant dimensions are selected (Fig 4A,B). Once planning is complete and has been approved, the digital plan is sent to the manufacturer for production of the guide using stereolithography. Stereolithography is an additive manufacturing process using a vat of liquid photopolymer resin, curable by ultraviolet light, and an ultraviolet laser that selectively cures resin, layer by layer, into a mass representing the desired three-dimensional object. For each layer, the laser beam traces a part cross-section pattern on the surface of the liquid resin. The supports are removed manually after the product is taken from the stereolithography machine (Fig. 6A). After this process, the sleeves for the drill keys are positioned in the guide. When the guide is finished, it is sent to the surgeon (Fig. 6B). Depending on the system, a list with an overview of the planned implants is included, as well as a patient-specific manual. As the markers are visible in both sets of scans, they can be transformed and realigned to fuse the prosthesis within the maxillofacial structures. Besides an adequate bone model, derived from scanning the patient with the denture in situ, the second scan allows optimal visualization of the prosthesis. Therefore, both models can be presented separately, allowing planning on the bone (Fig. 4A) and/or the prosthetic model. Moreover, by accurate fusion, whilst maintaining excellent image quality, the planning can be carried out and controlled toward the integrated model (Fig. 4B) (27, 58). Regardless of the method used, correct positioning of the scan prosthesis is very important. Therefore, an index is strongly recommended to position and stabilize the template in the mouth of the patient during the scanning process (Fig. 5). An optimal fit of the scan prosthesis with the patient’s soft tissue is crucial. One should determine whether air is visible between the scan prosthesis and the soft tissue. This is especially important for mucosa-supported guides, in which the basis of the future surgical guide will be the same as the basis of the scan prosthesis. The DICOM images are imported in a software program, fusion of the scan prosthesis via the markers is accomplished and the ideal surgical site and optimal implant dimensions are selected (Fig 4A,B). Once planning is complete and has been approved, the digital plan is sent to the manufacturer for production of the guide using stereolithography. Stereolithography is an additive manufacturing process using a vat of liquid photopolymer resin, curable by ultraviolet light, and an ultraviolet laser that selectively cures resin, layer by layer, into a mass representing the desired three-dimensional object. For each layer, the laser beam traces a part cross-section pattern on the surface of the liquid resin.
