2 Notas 1. Na literatura médica portuguesa o termo "doente" é aplicado comumente às pessoas utentes de um hospital, clínica ou sistema de saúde em geral. Adotando uma terminologia pró-saúde, baseada em medicina preventiva e não curativa, optou-se por chamar aos utentes "doentes" de "pacientes". 2. Resumo Ao longo dos anos a informatização dos registos médicos dos pacientes, através de sistemas de informação clínica, tem aumentado exponencialmente fazendo com que haja melhora de produtividade e qualidade no que diz respeito aos cuidados médicos proporcionados aos pacientes. Apesar dos avanços tecnológicos neste campo, ainda se verifica que a utilização de papel para o registo clínico dos pacientes continua a ser uma prática bastante comum dentro dos hospitais. Um dos motivos para esta situação deve-se ao facto de a maior parte dos sistemas de informação clínica abrangerem apenas conjuntos limitados de dados como, por exemplo, análises laboratoriais, registos de exames de diagnósticos específicos, entre outros. Para além do problema referido, a necessidade de dedicação e consumo de tempo pela parte dos utilizadores, são dois fatores que colocam um forte entrave na utilização das aplicações existentes. Mais especificamente, na área da Cardiologia, observa-se a existência de vários modelos de dados desenhados para algumas das suas subáreas. Mas, dadas as necessidades variarem de caso para caso, é possível encontrar sempre novos modelos de dados a ser implementados para melhorar os cuidados médicos a um paciente e também permitir melhorar a qualidade dos dados para análise retrospetiva. Numa colaboração com o Real Hospital Português (RHP) (Pernambuco, Brasil), sentiu-se a necessidade de criação de um modelo de dados que se adapte à rotina das consultas efetuadas em Cardiologia Pediátrica deste hospital e implementação de uma plataforma web que permita a análise da informação existente de pacientes individuais assim como a possibilidade do especialista construir consultas específicas para o Eletrocardiograma (ECG). O modelo de dados foi construído com base na bibliografia médica existente na área da Cardi3
4 ologia e refinada com o apoio de profissionais da área. Várias decisões efetuadas ao longo do projeto foram guiadas pelo tipo de consulta desejado pelos especialistas para cruzamento de dados relevantes. Alguns exemplos de consultas desejadas são: •Encontrar o número de pacientes com determinado tipo de patologia e residente em determinada região •Cruzamento entre os sintomas que levaram o paciente à consulta com o tipo de patologia resultante Além do modelo de dados, foi criada uma plataforma web que permite entrada de dados e consultas sobre os dados existentes. Ao proporcionar a entrada de dados, os registos em papel passam a ser armazenados em forma digital, além de permitir organizar e estruturar os dados de forma padrão evitando erros e inconsistências na informação armazenada. Através da funcionalidade de consultas é possível criar visões de cruzamento de informação para estudo e análise dos dados. O armazenamento dos dados permite ainda, que subconjuntos de dados possam ser exportados para efeitos de estudos estatísticos e de mineração. Abstract Over the years the computerization of the patient medical records, through clínical information systems, has increased exponentially so that there is an improvement in productivity and quality regarding to health care provided to the patients. Despite technological advances in this field, the use of paper for the patient medical records remains a common practice within hospitals. One reason for this is due to the fact that the majority of clínical information systems cover only limited sets of data, for example, laboratory analysis, specific diagnostic tests records, among others. In addition to the problem above, the need for dedication and time consumption by the users, are two factors that place a strong constraint on the use of existing applications. More specifically, in the area of Cardiology, there are various types of data models for some of its areas. But as the needs vary from case to case, one can always find new data models to be implemented to improve medical care and allow new discoveries to be made to improve medical care to a patient and also improve the quality of data for retrospective analysis. So, in collaboration with the RHP(Pernambuco, Brasil), we designed a data model that adapts to routine queries performed in the Pediatric Cardiology area of this hospital and implementated a web interface that allows the analysis of existing information for individual patients as well as the possibility of constructing expert queries specific to the ECG. The data model, was built after thorough medical bibliography research. We also had help from experts in the area. Several decisions made during the project were guided by the type of query desired by the experts to cross the relevant data. Some examples of desired queries are: 5
6 •Find the number of patients with a particular type of pathology and resident in a particular region •Crossing between the symptoms that led the patient to the appointment with the resulting type of pathology In addition to the data model, we created a web platform that allows data entry and queries on the data. By providing data entry, paper records will be stored in digital form, and will allow organizing and structuring data in a standard way avoiding inconsistencies and errors in the stored information. Through the query feature one can create visions of crossing information for study and analysis. CAPÍTULO 1. INTRODUÇÃO 14 Assim, os objetivos deste trabalho são os seguintes: 1. A recolha de atributos relevantes na área da Cardiologia 2. A construção e modelagem de dados com base nas variáveis recolhidas 3. A construção de uma base de dados com dados provenientes do RHP 4. A construção de uma plataforma web de entrada e consulta de dados voltada para a consistência e padronização dos dados para consulta A recolha de atributos baseou-se numa pesquisa exaustiva à literatura médica respetiva e na experiência e suporte da equipa médica especialista no RHP. A construção e modelagem dos dados foi realizada de maneira a tornar as consultas de rotina desejadas pelos especialistas mais simples e eficientes. Como exemplo, numa consulta de rotina pode-se pretender recuperar todas as consultas de um determinado paciente ou então, verificar quantos pacientes têm uma determinada patologia. Apesar de ter sido dado ênfase a toda a informação que envolve uma consulta de rotina, foi dada maior atenção ao ECG como exame de diagnóstico de maior importância para a deteção de patologias. Para popular a base de dados utilizou-se um conjunto de 19257 pacientes com alguma informação geral duma consulta de rotina e outra mais específica do ECG pois a vertente de consultas da plataforma está especializada para o ECG. Por fim, para a implementação da plataforma web recorreu-se a mockups apresentados aos especialistas antes da sua implementação. Desta forma, as alterações necessárias foram realizadas antes da implementação, evitando assim, um consumo de tempo mais elevado na construção da plataforma. Como contribuições deste trabalho encontra-se: •A recolha e organização sistemática das variáveis na área da Cardiologia geral e, em particular, em Cardiologia Pediátrica •Modelo de dados relacional construído com o intuito de uso em consultas em Cardiologia Pediátrica
CAPÍTULO 1. INTRODUÇÃO 15 •Plataforma web para entrada de dados e consultas De uma forma geral, este trabalho tem dois focos principais: 1. Foco clínico - Permite a recolha da informação resultante de uma consulta de rotina, neste caso em Cardiologia, de forma organizada e padronizada. Assim, para além de facilitar a interpretação dos relatórios por outros, possibilita diversos tipos de estudos sobre os mesmos. Mais ainda, o desenvolvimento de uma base de dados em Cardiologia, ao facilitar a integração e gestão de informação dos pacientes, vai possibilitar uma melhor qualidade na prestação de cuidados de saúde. 2. Foco de investigação - Permite a realização de inúmeros estudos na área de Cardiologia, mais especificamente no que diz respeito ao ECG, como foi referido anteriormente. A partir do momento em que os dados se encontram padronizados é garantidamente mais viável concretizar estudos que dificilmente seriam postos em prática na ausência de um modelo de dados. Esta dissertação está dividida em seis capítulos: introdução, estado da arte, bases de dados relacionais, construção de uma base de dados em Cardiologia, plataforma web e, conclusões e trabalhos futuros. Neste primeiro capítulo foi apresentada uma pequena introdução sobre o tema em geral, os objetivos do trabalho e as suas contribuições. No segundo capítulo são apresentados alguns trabalhos relacionados e discutidas boas características de um modelo de dados para Cardiologia. No terceiro capítulo é apresentado um preâmbulo que aborda bases de dados relacionais. No quarto capítulo é apresentada a construção da base de dados em Cardiologia. Este capítulo divide-se em três secções: as variáveis recolhidas em Cardiologia, o modelo de dados e a população da base de dados. Na primeira secção é detalhada a estruturação das variáveis em Cardiologia que foram recolhidas e a metodologia de recolha. Na segunda secção é apresentada a ferramenta utilizada, explicada a escolha da modelagem e são expostas algumas tabelas do modelo de dados. Por último, na terceira secção é discutida a metodologia utilizada para a população da base de dados.
CAPÍTULO 1. INTRODUÇÃO 16 No quinto capítulo é apresentada a plataforma web de entrada de dados e consultas em que, inicialmente são explicados os motivos de escolha das ferramentas utilizadas. Este capítulo divide-se em duas secções: mockups e estrutura e funcionalidades da plataforma web. Na primeira secção é apresentado um conjunto de figuras e respetivas explicações provenientes da utilização de uma interface gráfica de construção de mockups que ajudaram no processo de visualização da plataforma a ser desenvolvida. A secção de funcionalidades e estrutura, encontra-se dividida em três subsecções: a primeira para a entrada de dados, a segunda para consultas em pacientes específicos, e a última para o query builder para consultas que permitem o cruzamento de dados entre tabelas. No sexto capítulo são apresentadas as conclusões retiradas deste trabalho e trabalhos futuros possíveis.
Capítulo 2 Estado da Arte Para o estado da arte foi analisada literatura correspondente aos temas considerados fundamentais para este projeto. É de notar que o estudo efetuado para este capítulo, influenciou de alguma forma as decisões tomadas para este projeto. Em primeiro lugar é necessário identificar os temas fundamentais do projeto a ser desenvolvido. Dado que o objetivo é a construção de um repositório de casos clínicos em que, através de uma plataforma web, seja possível a análise dos mesmos e a construção de queries pela parte do utilizador, os focos principais são o modelo de dados e o query builder para as consultas dos dados. Assim sendo, para a pesquisa da literatura existente, foram utilizadas as seguintes bases de dados e respetivas queries de pesquisa: •IEEE Xplore [5]: medical information AND interface AND query (117 resultados devolvidos); medical information AND interface AND cardiology (132 resultados devolvidos); relational databases AND cardiology (59 resultados devolvidos) •Scopus [6]: medical information AND interface AND query AND cardiology (9 resultados devolvidos) •PubMed [7]: ("Information Systems"[Mesh]) AND "Cardiology"[Mesh] (282 resultados devolvidos); relational databases AND cardiology (5 resultados) 17
CAPÍTULO 2. ESTADO DA ARTE 18 Depois de introduzidas as queries de pesquisa, foi necessário utilizar algum método de exclusão de artigos. A metodologia utilizada foi a seguinte: •Leitura do título e abstract dos artigos tendo sido excluídos aqueles que não se enquadravam nos objetivos do projeto a ser realizado. É de realçar que grande parte dos resultados relativos a modelos de dados em Cardiologia foram excluídos visto que, se destinavam a subgrupos específicos desta especialidade, como por exemplo, serem relativos a uma patologia específica. O objetivo do projeto é possibilitar consultas de rotina de Cardiologia. •Teve de ser efetuada nova exclusão de artigos, apesar de estes, através do título e respetivo abstract, mostrarem bastante relevância, visto não estarem disponíveis. Devido a este fator, o conjunto de artigos resultante para o estado de arte em questão ficou bastante reduzido. Outro fator de exclusão poderia ser a data de publicação mas, dado apenas termos disponíveis muito poucos artigos que se enquadrassem realmente dentro do nosso projeto e a maioria ser dos anos 80/90, optou-se por não excluir estes resultados. Assim sendo, para este estado da arte, foram utilizados quatro artigos. No artigo "Generic data modeling for clínical repositories" [181], o autor pretende encontrar a melhor forma de construir um repositório clínico, através de um modelo de dados relacional, que capture todos os elementos vitais para os cuidados de saúde e que possibilite um acesso rápido aos dados dos pacientes. Dada a quantidade e complexidade do conhecimento e o número de intervenientes durante todo o processo que trata os cuidados médicos a proporcionar a um paciente, a modelagem dos dados envolventes é uma tarefa bastante difícil. Outra dificuldade é o facto de que o conhecimento existente, apesar de vasto, ainda é bastante incompleto, havendo muitas falhas em determinadas áreas. Este motivo faz com que o grau de formalização seja limitado. De forma a perceber melhor a metodologia utilizada para o desenho da base de dados, é importante saber o que o autor entende por repositório clínico e sistema de informação clínica. Dito isto: •Repositório clínico: Recurso partilhado dos dados dos pacientes, tanto ambulatórios como hospitalizados.
CAPÍTULO 2. ESTADO DA ARTE 19 •Sistema de informação clínica: Aplicação que coleciona, processa e exibe a informação existente no repositório clínico. A técnica utilizada para a base de dados a ser construída tem como objetivo permitir uma rápida recuperação da informação do paciente assim como, ser facilmente adaptável para a introdução ou alteração de informação. Assim sendo, a metodologia utilizada pelo autor para a construção do modelo de dados é a seguinte: 1. Construção de um modelo conceptual dos dados médicos que vão ser geridos pelo sistema de informação clínica. Este esquema deve ser construído utilizando termos que permitam aos especialistas da área médica perceber a sua estrutura com facilidade. 2. Filtragem de conceitos e relações que não variam nos registos dos pacientes visto que o interesse principal para o repositório clínico se encontra nos dados que variam de paciente para paciente. Dentro desta filtragem de dados encontra-se a informação explicativa sobre outros conceitos do esquema em que, estes dados não variam de paciente para paciente. Como exemplo, no esquema apresentado no artigo, nas análises clínicas, são filtradas as substâncias medidas e o tipo de amostra medida ficando apenas o nome do conceito no modelo (isto é, no caso do conceito Serum-Sodium-Test, diz respeito a um teste que mede os níveis de sódio numa amostra de sangue). Concluindo, o esquema vai ser dividido em duas partes: um repositório clínico e um dicionário de dados clínicos para manter a informação semântica sobre as análises clínicas, medicamentos, entre outros. É esta solução que vai permitir maior eficiência na recuperação de informação de pacientes individuais e a adaptação a alterações nas aplicações clínicas. 3. Transformação do esquema detalhado num esquema genérico obtido após a eliminação dos dados que não variam de paciente para paciente. Ou seja, ao adaptar conceitos muito detalhados em conceitos genéricos, por sua vez, vai dar origem a um esquema também genérico. 4. Implementação de um esquema genérico utilizando um sistema de gestão de bases de dados em que cada conceito diz respeito a uma tabela da base de dados da seguinte forma:
CAPÍTULO 2. ESTADO DA ARTE 20 (a) As relações de um determinado conceito são colunas da tabela a que dizem respeito com o tipo de dados apropriado. (b) A chave da tabela ou é a coluna correspondente ao identificador único do conceito ou, é alguma combinação de colunas que determinam uma linha única da tabela. (c) Criação de chaves estrangeiras que relacionam dois conceitos. Tendo em consideração que o foco da base de dados são os eventos de um paciente, a maior parte das tabelas tem uma coluna que identifica cada um dos pacientes que pode ser usada para: 1. Indexar linhas das tabelas proporcionando acesso rápido a informações sobre o paciente, através do respetivo identificador. 2. Armazenamento físico de grupos de linhas para melhorar o tempo de consulta de um paciente reduzindo as páginas de disco a serem examinadas pelo sistema de gestão da base de dados. 3. Distribuição dos dados em vários discos permitindo consultas sobre pacientes diferentes executadas em paralelo. 4. Distribuição dos dados em vários servidores reduzindo a carga de transação de um dado servidor. Em "Data for cardiovascular modeling" [182] os autores abordam as vantagens e limitações existentes na modelagem dos tipos de dados que envolvem doenças cardiovasculares. Inicialmente, os autores descrevem quais os tipos de bases de dados existentes dentro desta categoria da área médica: •Bases de dados respeitantes a pacientes portadores de um seguro que paga as despesas do serviço. Estas, são limitadas a variáveis relativas à idade, sexo, diagnósticos, procedimento, tempo de permanência e estado de alta. •Bases de dados de ensaios clínicos randomizados que proporcionam informação focada e detalhada. Estas são direcionadas para apenas uma comparação de terapia de pacientes específicos.
CAPÍTULO 2. ESTADO DA ARTE 21 •Bases de dados relativas a um procedimento ou diagnóstico. •Bases de dados de gestão administrativa ou de cuidados médicos. •Bases de dados clínicas, em que estas são formadas através dos dados dos pacientes que são rotineiramente registados. •Bases de dados de arquivo criadas para utilização em estudos de investigação retrospetiva, ajudando a identificar grupos de pacientes. •Bases de dados de análise de qualidade. Para a modelagem de dados, é necessário avaliar as bases de dados em função dos objetivos. Estas podem ser usadas tanto para pacientes individuais como para um conjunto de pacientes. Alguns dos desafios da modelagem deste tipo de dados passam pela união de registos de pacientes em que estes se podem encontrar em bases de dados diferentes (sendo este um dos grandes problemas existentes), pela quantidade de dados existentes, pela consistência da informação ao longo do tempo, entre outros. Uma forma de combater o problema de junção de dados de diferentes bases de dados passa pela utilização de definições standard para resolver se e em que medida os dados podem ser fiáveis. Numa base de dados para doenças cardiovasculares, encontram-se muitas categorias de informação: demográfica, dados históricos e físicos, procedimentos de diagnóstico, tratamentos, resultados, custos e encargos, fornecedores e qualidade de vida. Na época em que este artigo foi publicado (1989), não existia nenhuma base de dados que tivesse reunido toda esta informação. E, atualmente, em função da pesquisa efetuada, continua a não existir nenhum modelo de dados que inclua toda a informação que envolve doenças cardiovasculares. Concluindo, apesar da necessidade de existência de um modelo para doenças cardiovasculares ainda não foi encontrada solução completamente eficiente para este problema. Em "Use of an Electronic Medical Record in a Department of Cardiology" [179] os autores estudam o impacto do uso de um sistema de registos clínicos especificamente num departamento de Cardiologia. O sistema utilizado foi definido para a utilização em várias unidades e diferentes serviços de cuidados médicos para haver um acesso aos dados administrativos e clínicos dos pacientes. Este sistema está dividido em três subsistemas: administrativo, de diagnóstico e clínico. Desta forma, foi permitida a integração de vários tipos de dados
CAPÍTULO 2. ESTADO DA ARTE 22 no departamento de cardiologia. Após este sistema estar em uso no Instituto de Fisiologia Clínica em Pisa, foram encontrados alguns problemas entre os quais se encontra a eficácia da interface e o tempo necessário para aceder aos dados. Assim que os problemas encontrados foram resolvidos, a utilização deste sistema foi compensadora. Algumas das vantagens da sua utilização foram: •A prescrição médica passou a ser mais precisa evitando erros na medicação estabelecida •A informação trocada passou a ser também mais precisa •Uma análise completa e detalhada dos custos •Melhorias nos cuidados médicos devido à disponibilização de toda a informação do paciente No artigo "Electronic Medical Records and Medical Research Databases - Can They Be Synonymous?" [183], os autores abordam os fatores que envolvem a utilização de um sistema de gestão de registos médicos como um recurso de informação da rotina médica dos pacientes com a capacidade de possibilitar investigação médica sobre os dados nele incluído. O sistema proposto neste artigo baseia-se na divisão dos dados recolhidos em duas unidades lógicas: uma privada e uma pública. Esta particularidade tem como objetivo proteger a identidade do paciente. Como para a rotina clínica de cada paciente é necessária a recolha de informação privada, esta solução permite "esconder" essa informação de forma que apenas os campos relativos à informação clínica sejam visíveis quando entramos no campo da investigação. Os potenciais benefícios através da utilização de um sistema que tenha em atenção esta particularidade são indiscutíveis. Todas as imagens médicas, análises clínicas, diagnósticos, entre outros, fariam imediatamente parte do registo público de investigação. Os artigos estudados neste capítulo apresentam questões e soluções relevantes a este trabalho, dentro das quais se ressaltam: 1. A utilização de bases de dados relacionais é considerada uma boa solução para a construção de um repositório clínico e implica algumas noções a serem aplicadas durante o seu desenho. Como exemplos encontram-se a criação de chaves para identificar univocamente elementos de uma tabela e a criação de outras chaves (chaves estrangeiras) para
CAPÍTULO 2. ESTADO DA ARTE 23 relacionar tabelas. 2. O modelo de dados deve ser adequado aos objetivos. 3. Os especialistas devem facilmente compreender a estrutura do modelo de dados. 4. O modelo de dados deve permitir uma recuperação rápida da informação do paciente assim como estar desenhado de maneira a ser facilmente adaptável à introdução ou alteração de informação. 5. O repositório pode ser implementado de forma a ser utilizado tanto a nível hospitalar como em investigação através da capacidade de "esconder" a informação privada do paciente quando fosse utilizado a nível de investigação.
CAPÍTULO 4. CONSTRUÇÃO DE UM MODELO DE DADOS EM CARDIOLOGIA 30 e por Email. Foi muito importante este acompanhamento porque, desta forma, o modelo de dados foi construído de maneira a que, no final, a interface de consultas, tivesse o desempenho pretendido. Este capítulo é constituído por três secções: uma secção sobre as variáveis em Cardiologia recolhidas, uma secção relativa ao modelo de dados construído e, por último, uma secção sobre a integração de dados na base de dados. 4.1 Variáveis em Cardiologia A recolha de variáveis para a construção do modelo de dados foi uma fase de grande importância para este trabalho. Assim, a metodologia utilizada para a escolha e recolha das variáveis foi a seguinte: 1. Numa primeira fase elaborou-se uma leitura detalhada de alguns capítulos de livros no âmbito da Cardiologia [185], Semiologia [184] e de um guia para exames físicos e recolha da história clínica [178]. Foi necessário ter em consideração vários fatores durante o processo de seleção de variáveis. Entre eles, os mais relevantes foram a organização e seleção de toda a informação existente, que vão ser explicados com detalhe em seguida. É importante referir que a tarefa de organização e seleção de dados foi elaborada de forma a padronizar a informação recolhida durante uma consulta médica de Cardiologia. Houve também a preocupação de modelar os dados de forma que fosse possível efetuar consultas inteligentes para melhorar a prática médica e permitir estudos retrospetivos. Assim sendo, de forma a ser possível a padronização da informação recolhida numa consulta médica em Cardiologia Pediátrica e, efetuar consultas inteligentes, decidimos que a recolha de informação para o modelo de dados, além de se basear nas boas práticas sugeridas nos trabalhos mencionados no capítulo 2, seria direcionada em função dos nossos objetivos. Apesar da interface a ser construída se destinar à Cardiologia Pediátrica, inicialmente, as variáveis recolhidas, foram referentes a uma consulta em Cardiologia geral. Portanto, o primeiro passo neste estudo foi perceber quais as secções presentes numa consulta
CAPÍTULO 4. CONSTRUÇÃO DE UM MODELO DE DADOS EM CARDIOLOGIA 31 médica em Cardiologia (de notar que estes são os mesmos para consultas de qualquer especialidade), designadas por anamnese. É bastante importante referir que, durante uma consulta, os pontos que se encontram na literatura não são seguidos rigorosamente. Foi efetuada uma adaptação da informação encontrada na literatura em função daquilo que realmente é questionado e anotado durante uma consulta. Assim, a estrutura formal da anamnese foi definida da seguinte forma: (a) Informação do paciente - Todos os campos necessários para a identificação do paciente. (b) História clínica atual - Conjunto de sintomas que levaram à consulta. (c) História médica passada - Episódios resolvidos do paciente. (d) Caracterização do estado de saúde - Episódios ativos ou passivos que influenciam o estado atual de saúde do paciente (e) História psicossocial - Informação relativa à vida do paciente. (f) História familiar - Informação médica considerada importante sobre os familiares mais próximos do paciente. (g) Exame físico - Conjunto de técnicas utilizadas pelos especialistas durante uma consulta em Cardiologia. É importante referir que a recolha efetuada pode não conter todas as técnicas existentes. (h) Exames de diagnóstico - Conjunto de exames de diagnósticos possíveis em Cardiologia. Da mesma forma que no caso anterior é possível que não estejam incluídos todos os exames de diagnóstico existentes em Cardiologia. (i) Exames complementares de diagnóstico - Análises clínicas possíveis que podem ser requisitadas pelo médico de forma a ajudar no diagnóstico. É de realçar que não foram recolhidos todos os exames complementares de diagnósticos existentes visto que este conjunto atinge proporções demasiado elevadas e não são um fator de importância maior para o objetivo final. Para além da anamnese foi necessário recolher informação relativamente ao diagnóstico de um paciente. O segundo passo foi separar a informação encontrada e colocá-la na secção correspondente utilizando o Microsoft Office Excel como ferramenta. Para tal, mais uma vez se
CAPÍTULO 4. CONSTRUÇÃO DE UM MODELO DE DADOS EM CARDIOLOGIA 32 efetuou um estudo na literatura de forma a recolher o máximo de informação possível. Apesar da quantidade substancial de dados recolhidos, não se encontra presente toda a informação existente mas, dada a limitação de tempo e a informação essencial estar presente, optou-se por não continuar a pesquisa. Relativamente ao conjunto de exames existentes, o ECG foi aquele que teve maior atenção da nossa parte dado corresponder ao exame de diagnóstico mais importante em Cardiologia. 2. De seguida, através de reuniões com a equipa de especialistas foi efetuado um refinamento das variáveis e valores recolhidos de forma a serem direcionadas da melhor forma possível para consultas em Cardiologia Pediátrica em que foram tomadas decisões como inclusão/exclusão de variáveis e/ou valores. As tabelas construídas encontram-se detalhadas no Apêndice A. 4.2 Modelo de Dados Para esta fase do projeto, foi necessária a escolha de uma ferramenta que permitisse a modelagem dos dados recolhidos. Das alternativas disponíveis, foram selecionadas as de software livre. Desta forma, optou-se pela utilização do MySql Workbench [11], tendo sido utilizada a edição MySQL Workbench Community Edition dado ser open source. A ferramenta escolhida para além de permitir o desenvolvimento gráfico do modelo de dados pretendido, possibilita popular a base de dados, a execução de queries SQL sobre estes e por fim, a utilização de Forward Engineer para criar um ficheiro .sql com as tabelas criadas e respetivos valores. Visto a plataforma a ser desenvolvida ser direcionada para uso exclusivo do RHP, pareceu lógico que as tabelas da base de dados fossem desenhadas de maneira a ir ao encontro do tipo de consultas a que esta se destinava. Para tal, nesta fase, foi necessário bastante acompanhamento por parte dos médicos especialistas em Cardiologia Pediátrica. Como foi dito na secção anterior, o conjunto de variáveis correspondente às análises clínicas não traz grande relevância para este trabalho. Apesar de incompleto (dada a quantidade

CONSTRUÇÃO DE UM MODELO DE DADOS EM CARDIOLOGIA 33 extensa de análises clínicas existentes) este conjunto encontra-se bastante complexo, portanto, tendo também em consideração o prazo de entrega do projeto, pareceu bastante plausível não considerar esta informação. Após a escolha da ferramenta a utilizar foi desenhado um modelo em que apenas foram apresentados os grupos referidos na secção anterior e respetivos relacionamentos (Figura 4.1) permitindo assim, apresentar aos especialistas uma visão geral que estes compreendessem do tipo de relacionamento existente numa consulta de rotina em Cardiologia para um paciente. Figura 4.1: Visão geral do modelo de dados de uma consulta de rotina em Cardiologia para um paciente De seguida, foi necessário decidir qual a melhor forma de estruturar a base de dados (tendo em conta o processo de normalização) finalizando com a sua implementação. Antes de detalhar a forma como foi desenvolvido o modelo de dados, é necessário introduzir a nomenclatura utilizada para as chaves das tabelas: •PK - A nomenclatura das PK foi definida utilizando o nome da tabela seguido de ID. Como exemplo temos a PK da tabela information:informationID. •FK - A nomenclatura das FK foi definida utilizando o nome da tabela com a qual existe relacionamento seguida do nome da PK respetiva. Como exemplo temos uma das FK da tabela family que está relacionada com a tabela appointment que por sua vez tem na sua estrutura a PK information_informationID. Como resultado, obtemos a FK appointment_information_informationID. A única exceção à metodologia usada encontra-se na tabela appointment. Para identificar esta tabela são utilizadas duas chaves primárias em que uma delas é resultante de outra tabela.

Como esta tabela se encontra relacionada com a tabela information, a PK resultante é: information_informationID. Para a recolha de variáveis foram consultadas as referências entre [12] e [13], inclusive. Nas secções seguintes vão ser apresentadas e explicadas as entidades, e respetivas relações, que constituem o modelo de dados desenvolvido. Na Figura 4.2, é possível observar uma parte do modelo de dados. Não vão ser expostas todas as tabelas, dado o conjunto total das mesmas ser relativamente grande e, o raciocínio ser semelhante em grande parte delas. No Apêndice B encontra-se o modelo de dados completo, apresentando numa figura apenas os nomes das tabelas e respetivos relacionamentos e, nas figuras seguintes as restantes tabelas (e devida informação) que não estão incluídas na figura.

CONSTRUÇÃO DE UM MODELO DE DADOS EM CARDIOLOGIA 36 4.2.1 Identificação das entidades Nesta secção vão ser apresentadas todas as entidades que constituem o modelo de dados, onde é efetuado um relacionamento entre a estrutura apresentada na secção anterior e as entidades: 1. Informação do paciente: •information - Entidade que contém a informação básica de um determinado paciente. Ou seja, a informação que não varia ao longo do tempo. •appointment - Entidade principal da base de dados, que diz respeito a uma consulta em específico de um determinado paciente, cuja informação básica se encontra em information. Esta entidade é o núcleo do modelo de dados visto que quase todas as outras entidades estão diretamente ligadas a esta. 2. História clínica atual: •symptoms - Entidade correspondente aos motivos que levaram o paciente à consulta, ou seja, a sua história clínica. •symptomsInfo - Entidade que contém um conjunto de informações sobre cada um dos sintomas que levaram o paciente à consulta. 3. História médica passada: •pmh_general - Entidade correspondente à informação de carácter geral no que diz respeito à fase da história médica passada do paciente. •pmh_women - Entidade que apenas se destina a pacientes do sexo feminino, contendo a informação de história médica passada ginecológica e obstétrica dos mesmos. 4. Caracterização do estado de saúde: •chc_general - Entidade correspondente à informação de carácter geral no que diz respeito à fase de caracterização do estado de saúde. •chc_routineTests - Entidade correspondente aos testes de rotina considerados relevantes na caracterização do estado de saúde.

Diagnóstico: •diagnosis - Entidade correspondente ao diagnóstico do paciente. É importante referir que algumas das fases da anamnese foram divididas em duas ou mais entidades. Esta decisão provém das decisões de estruturação da base de dados para que esta seja adaptada da melhor forma possível aos objetivos pretendidos. •Caracterização do estado de saúde - Neste caso optou-se por dividir este tópico em três entidades: chc_general,chc_routineTests ert_dates. O motivo para esta decisão passa pela necessidade de termos disponível as datas de realização de testes de rotina e a melhor forma encontrada para tal foi a divisão desta fase da anamnese em dois de forma a, de seguida, criar uma entidade correspondente às datas dos exames efetuados relacionada com a entidade chc_routineTests. •História médica passada - Este tópico foi também dividido em duas entidades: pmh_general epmh_women. Dado que aqui se encontrava informação correspondente apenas a pacientes do sexo feminino, decidimos então, dividir esta fase em dois de forma a termos uma entidade com toda a informação geral (pmh_general) e outra que será apenas direcionada a pacientes do sexo feminino (pmh_women). Desta forma a tabela pmh_women apenas é preenchida no caso de o paciente ser do sexo feminino evitando gastos desnecessários de memória. •Exames físicos - No que diz respeito aos exames físicos optou-se por separar cada um deles numa tabela específica. Desta forma, a base de dados fica mais organizada e adaptada aos objetivos finais além de tornar as consultas à base de dados mais diretas e

Como o médico em princípio não vai requisitar todos os exames existentes na base de dados numa consulta, este tipo de esquema evita que muitos dos campos sejam guardados ocupando inclusive, espaço de memória desnecessário. •Exames de diagnóstico - As decisões tomadas para os exames de diagnóstico vão de encontro ao tópico anterior. 4.2.2 Identificação dos relacionamentos Depois de identificar todas as entidades existentes no modelo de dados, é importante perceber como é que estas se relacionam entre elas. Assim sendo, nesta secção, vão ser apresentados todos os relacionamentos que fazem parte da base de dados. •information:appointment - Relação do tipo 1:N. Ou seja, a um paciente podem estar associadas mais do que uma consulta. •appointment:symptoms - Relação do tipo 1:N. Ou seja a cada consulta, um paciente pode ter mais do que um sintoma presente. •symptoms:symptomsInfo - Relação do tipo 1:1. É apresentada informação específica relativa a cada um dos sintomas. Isto é, a cada sintoma corresponde apenas um conjunto de informação específica. •appointment:chc_general - Relação do tipo 1:1. Ou seja, uma consulta apenas pode ter uma caracterização do estado de saúde (neste caso, de carácter geral). •appointment:chc_routineTests - Relação do tipo 1:N. Ou seja, um paciente pode ter efetuado mais do que um exame de rotina numa consulta. •appointment:rt_dates - Relação do tipo 1:1. São apresentadas datas específicas relativas a cada um dos exames de rotina. Isto é, a cada exame de rotina corresponde apenas uma data específica. •appointment:psicoSocial - Relação do tipo 1:1. Ou seja, cada consulta apenas pode ter uma história psicossocial.

Capítulo 5 Plataforma Web Depois de definida a base de dados e de esta ter sido populada com os respetivos dados, a próxima fase é o desenvolvimento da plataforma Web que dá suporte à base de dados desenvolvida. Para a sua implementação, optou-se pela utilização da linguagem PHP [14]. Os motivos para esta escolha passaram pelo facto de esta ser bastante utilizada para desenvolvimento web encontrando-se especialmente equipada para o desenvolvimento deste tipo de aplicações, sendo possível embuti-la na linguagem de marcação HTML [15]. Além disso suporta o MySQL que é software livre. A plataforma Web tem três objetivos principais: entrada de dados durante a consulta de rotina de um paciente, a procura de um paciente em específico apresentando os dados correspondentes às respetivas consultas de rotina e a utilização de um Query Builder que permita ao utilizador a construção de queries para consultas sob o exame de diagnóstico ECG em que os resultados podem ser apresentados sob a forma de tabela ou gráfico, conforme o solicitado. Neste capítulo, numa primeira fase, vão ser apresentados alguns mockups desenhados como suporte assim como os motivos que levaram à sua utilização. De seguida é descrita a estrutura e respetivas funcionalidades da plataforma, apresentando, por fim, alguns screenshots de views específicas. 49

PLATAFORMA WEB 50 5.1 Mockups Antes de proceder ao desenvolvimento da plataforma propriamente dita, foi utilizada uma interface gráfica de construção de mockups, o Balsamiq Mockups [16] tendo como objetivo o desenho do aspeto gráfico da plataforma. Esta fase, teve como objetivo final, durante o desenvolvimento efetivo da plataforma web, apesar de não se seguir fielmente o mockup desenhado, existir presente um guia de utilização pelos utilizadores. Desta forma, evita-se recorrer a mudanças, possivelmente constantes, ao longo da sua implementação. Todas estas decisões foram discutidas com a médica especialista em Cardiologia Pediátrica que tem acompanhado o projeto ao longo das várias fases. É importante referir que estes mockups apenas serviram como base, de forma a se perceber quais seriam os resultados a ser apresentados. Para a inserção de dados na base de dados desenhada, a plataforma web vai apresentar um conjunto de formulários relativos aos vários tópicos de uma consulta de rotina em Cardiologia construídos de forma a otimizar o tempo de preenchimento pela parte do médico. Na Figura 5.1 é apresentado como exemplo o formulário com a informação básica do paciente. Desta forma, o médico pode ir selecionando o tópico que pretende preencher sem ter de seguir uma ordem específica ou, até mesmo excluir tópicos
CAPÍTULO 5. PLATAFORMA WEB 51 que não sejam necessários na consulta a decorrer. Como exemplo, na Figura 5.2 é apresentado o formulário para a história psico-social do paciente. Figura 5.2: Mockup relativo à inserção de dados da história psicossocial de um paciente Caso o médico pretenda realizar a consulta de um paciente em específico, a plataforma web terá, de alguma forma, de possibilitar a procura do paciente cuja(s) consulta(s) pretende analisar. Como se pode observar pela Figura 5.3, selecionado o paciente, é apresentada a sua informação básica seguida de uma lista de todas as consultas efetuadas. Figura 5.3: Mockup relativo às consultas efetuadas por um respetivo paciente De seguida será apresentada uma lista com todos os tópicos que constituem a consulta de rotina para que o utilizador possa selecionar qual ou quais pretende analisar. Esta lista será apresentada sempre que o utilizador se encontre em algum destes tópicos para que a sua pesquisa seja efetuada de forma prática. No caso da Figura 5.4, foi selecionado o exame de diagnóstico ECG onde se podem observar as duas partes constituintes deste exame: o estudo anatómico e o estudo hemodinâmico.

Por último, para a realização de consultas sobre o exame de diagnóstico ECG, da mesma forma que para a consulta de um paciente em especifico, na página inicial o utilizador poderá selecionar esta opção. Depois de selecionada, será apresentado um query builder que permite ao utilizador construir a query desejada de forma simples (Figura 5.5). Construída a query, basta executá-la para que o resultado seja apresentado (exemplo de resultado no mockup da Figura 5.6). Figura 5.5: Mockup relativo ao query builder

PLATAFORMA WEB 53 Figura 5.6: Mockup relativo ao resultado da query Estes mockups foram revisados e refinados para dar origem à interface real apresentada na próxima secção. 5.2 Estrutura e Funcionalidades da plataforma web Como foi mencionado anteriormente, a plataforma web tem três vertentes: a inserção das consultas de um paciente, a pesquisa das consultas e respetivos exames efetuados por um determinado paciente e a apresentação de um query builder que permite ao especialista a construção e análise de consultas específicas. Na página inicial, como se pode observar pela Figura 5.7, é apresentada ao utilizador uma lista, na qual, é possível selecionar uma de três opções: inserção de um paciente, pesquisa por paciente ou o query builder. PLATAFORMA WEB 54 Nas secções seguintes, é descrita a forma como utilizar cada uma destas opções assim como alguns screenshots das mesmas. 5.2.1 Inserção de um paciente Caso o especialista pretenda inserir a informação relativa a uma consulta de rotina efetuada a um paciente, após a escolha desta opção na página principal, são apresentadas duas opções (Figura 5.8): inserir um paciente novo ou inserir uma consulta nova a um paciente que já se encontra na base de dados. Figura 5.8: Opções para a escolha de inserção de um paciente Caso escolha a primeira opção, é necessário ainda escolher se pretende inserir um paciente novo ou acrescentar uma consulta a um paciente que se encontra na base de dados. Caso escolha a primeira opção, é apresentado um formulário onde pode preencher os campos relativos à informação básica do paciente. Depois de inseridos estes dados na base de dados, segue-se o formulário para a informação geral de uma consulta. É importante referir que, no total dos formulários implementados, apenas dois campos foram considerados obrigatórios: nome e data da consulta. Dado que o preenchimento obrigatório de vários campos poderia ser maçador, optou-se por dar a hipótese ao especialista de decidir quais os campos a preencher durante a consulta. O motivo pelo qual se considerou os dois campos referidos como obrigatórios, diz respeito à forma como a plataforma para consultas por paciente foi desenvolvida. Para esta ser possível, é necessária a introdução do nome do paciente, seguida da data da consulta a analisar, como vai ser explicado na secção seguinte.
CAPÍTULO 5. PLATAFORMA WEB 55 Na Figura 5.9, é apresentado um screenshot do primeiro formulário (informação do paciente que não varia ao longo do tempo) a ser preenchido.
