Abstract

Las disciplinas psicosomáticas orientales, como el yoga y la meditación mindfulness, han sido un instrumento muy utilizado en la cultura Oriental, desde tiempos remotos, por los numerosos beneficios para la salud atribuidos a su práctica. El yoga proviene de la antigua cultura india y su práctica implica el desarrollo físico, mental y espiritual. La meditación mindfulness proviene de las prácticas budistas Vipassana y Zen y su práctica implica concentrarse en el momento presente de forma activa y plena, siendo totalmente conscientes de lo que estamos haciendo, pensando o sintiendo. El objetivo del presente estudio fue investigar los efectos de la práctica de yoga y meditación mindfulness sobre parámetros psicobiológicos, tanto en practicantes experimentados en yoga, como en pacientes con síntomas de ansiedad y depresión sometidos a un breve programa de meditación mindfulness. Para el estudio de yoga fueron reclutados veintiséis pacientes, estando el grupo experimental formado por practicantes habituales de esta disciplina. Todos los sujetos cumplimentaron el cuestionario Pittsburgh Sleep Quality Index (PSQI), para evaluar la calidad subjetiva de sueño, y se analizaron parámetros hormonales del eje HHA (ACTH, cortisol y DHEA-S). En los otros dos estudios de meditación mindfulness, dieciséis pacientes fueron sometidos a un programa de dos meses de duración. En estos casos se evaluó antes y después del programa de meditación, el bienestar psicológico (cuestionario Mental Health, MH-5), y determinados parámetros hormonales (ACTH, cortisol, DHEA-S, TSH, T4, T3 y PTH) e inmunitarios (recuento celular de la serie blanca, niveles de IgA, IgG, e IgM y los factores del complemento C3 y C4). Los resultados en el estudio de yoga mostraron, en el grupo experimental, puntuaciones más bajas en PSQI y niveles de cortisol en sangre más altos que en el grupo control. En los estudios de meditación mindfulness, los resultados en el post-test revelaron una mejora en la puntuación MH-5, mientras que se elevaron los niveles de DHEA-S y ACTH, y los niveles de PTH disminuyeron. Además, demostraron un aumento significativo en las concentraciones de IgG, IgM, C3 y C4, así como un descenso en el recuento de monocitos. Por tanto, la práctica habitual de yoga se asocia con importantes cambios psicobiológicos, incluyendo una mejor calidad del sueño, así como una acción moduladora sobre los niveles de cortisol. Al mismo tiempo, nuestros resultados sugieren que la meditación mindfulness puede ejercer una modulación psicobiológica significativa, a través de la mejora del bienestar psicológico, así como de la regulación en los niveles de DHEA-S, ACTH y PTH. Asimismo, parece ejercer una acción inmunomoduladora en pacientes con ansiedad y depresión. Por lo tanto, podemos concluir que las prácticas psicosomáticas orientales, como el yoga y la meditación mindfulness, pueden ejercen un efecto modulador entre los sistemas nervioso, endocrino e inmunitario, y muestran un interesante y potencial uso en la práctica clínica.

DOI:10.1016/j.biopsycho.2009.03.008 Abstract Yoga represents a fascinating mind–body approach, wherein body movements (asana), breathing exercises (pranayama) and meditation are integrated into a single multidimensional practice. Numerous beneficial mental and physical effects have been classically ascribed to this holistic ancient method. The purpose of the present study has been to examine the effects of long-term yoga practice on Subjective Sleep Quality (SSQ) and on several hormonal parameters of the hypothalamus–pituitary–adrenal (HPA) axis. Twenty-six subjects (16 experimental and 10 controls) were recruited to be part of the study. Experimental subjects were regular yoga practitioners with a minimum of 3 years of practice. Blood samples for the quantification of adrenocorticotropic hormone (ACTH), cortisol and dehydroepiandrosterone sulphate (DHEA-S) were drawn from all subjects. Likewise, the Pittsburgh Sleep Quality Index (PSQI) was employed to assess SSQ. As statistical analysis, Mann–Whitney U-test was performed. The yoga group displayed lower PSQI scores and higher blood cortisol levels than control subjects. Therefore, it can be concluded that long-term yoga practice is associated with significant psycho-biological differences, including better sleep quality as well as a modulatory action on the levels of cortisol. These preliminary results suggest interesting clinical implications which should be further researched. 2. DOI:10.1002/smi.1334 Abstract Mindfulness meditation is an ancient and simple form of meditation that has been said to induce several important physical and psychological benefits. The present study was designed with the aim of investigating the psychobiological effects of mindfulness meditation practice in a clinical population. Sixteen patients with depression and anxiety symptoms were submitted to a mindfulness meditation programme for the period of 2 months. Psychological wellbeing [5item Mental Health (MH-5)] and several endocrine parameters [adrenocorticotropic hormone (ACTH), cortisol, dehydroepiandrosterone-sulphate (DHEAS), thyroid-stimulating hormone, triiodothyronine, thyroxine and parathyroid hormone (PTH)] were assessed before and after the meditation programme. At post-test, the MH-5 score improved, while the levels of DHEAS and ACTH were raised, and those of PTH decreased. Our results suggest that mindfulness meditation can exert a significant psychobiological modulation by enhancing psychological wellbeing, as well as by regulating the levels of several hormonal parameters on different axes. 3. El objetivo del presente estudio fue investigar los efectos de esta forma de meditación sobre parámetros inmunitarios en pacientes con síntomas de ansiedad y depresión. Dieciséis pacientes fueron sometidos a un programa de meditación mindfulness de dos meses de duración. Diversos parámetros inmunitarios, como el recuento de leucocitos, linfocitos, monocitos, neutrófilos, eosinófilos y basófilos, así como los niveles de inmunoglobulinas (IgA, IgG, e IgM) y del complemento (C3 y C4), fueron evaluados antes y después del programa. Los resultados en el post-test revelaron un aumento significativo de las concentraciones de IgG, IgM, C3 y C4, así como un descenso en el recuento de monocitos. Estos hallazgos indican, por tanto, que la práctica de la meditación mindfulness parece ejercer una acción inmunomoduladora en pacientes con ansiedad y depresión, lo que sugiere interesantes implicaciones psicobiológicas y clínicas.
