Me hods and Resul s-—Hemodynamics we e eco ded om 167 men and 167 women o ma ching age (45 yea s) and body mass index (26.5) du ing passi e head-up il . None had diabe es melli us o ca dio ascula disease o he han hype ension o used an ihype ensi e medica ion. Whole-body impedance ca diog aphy, onome ic adial blood p essu e, and hea a e a iabili y we e analyzed. Resul s we e adjus ed o heigh , smoking, alcohol in ake, mean a e ial p essu e, plasma lipids, and glucose. Supine hemodynamic di e ences we e mino : Men had lowe hea a e (4%) and highe s oke index (+7.5%) han women (P<0.05 o bo h). Up igh sys emic ascula esis ance was lowe (10%), bu s oke index (+15%), ca diac index (+16%), and le ca diac wo k we e clea ly highe (+20%) in men han in women (P<0.001 o all). Co esponding esul s we e obse ed in a subg oup o men and pos menopausal women (n=76, aged >55 yea s). Hea a e a iabili y analyses showed highe low:high equency a ios in supine (P<0.001) and up igh (P=0.003) posi ions in men. Conclusions-—The o emos di e ence in ca dio ascula egula ion be ween sexes was highe up igh hemodynamic wo kload o he hea in men, a finding no explained by known ca dio ascula isk ac o s o ho monal di e ences be o e menopause. Hea a e a iabili y analyses indica ed highe sympa ho agal balance in men ega dless o body posi ion. The main findings we e e ified in a la ge g oup composed o an addi ional 313 men and 231 women. A o al o 480 men and 398 women we e examined. Me hods S udy Pa icipan s All pa icipan s we e in an ongoing s udy o hemodynamics in olun ee s, wi h he p ima y aim o examining he hemody- namic changes in p ima y and seconda y hype ension and no mo ensi e con ol pa icipan s in supine and up igh posi ions (DYNAMIC s udy; ClinicalT ials.go iden ifie NCT01742702). By he ime o he p esen analysis, 878 pa icipan s (480 men and 398 women) had unde gone hemodynamic mea- su emen s; o hose, 615 we e wi hou medica ions wi h di ec influence on ca dio ascula unc ion. The emaining pa icipan s included medica ed pa ien s wi h hype ension, ch onic enal disease, diabe es melli us, and a he oscle o ic ascula disease. All pa icipan s we e in e iewed o eco d li es yle habi s, amily his o y, and medical his o y. Clinical examina ion o ca dio ascula s a us and ou ine labo a o y es s we e pe o med. Fo he main analysis o he p esen s udy, pa icipan s wi h an ihype ensi e medica ion, co ona y hea disease, o p e ious myoca dial in a c ion, diagnosis o diabe es melli us, ca diac insu ficiency, a he oscle o ic ascula disease, enal disease, o ce eb o ascula disease we e excluded. In addi- ion, all pa icipan s using medica ions wi h po en ial influ- ence on hemodynamics (eg, a 1 -ad enocep o blocke s o p os a e p oblems, b 2 -ad enocep o agonis s, digoxin, and opical b-blocke s o glaucoma) we e excluded. 25 To a oid con ounding caused by di e ences in age and body weigh , 26– 29 he ollowing inclusion p o ocol was applied: A emale pa icipan was chosen, ollowed by selec ion o a male pa icipan wi h co esponding age (di e ence in age ≤3 yea s) and BMI (di e ence ≤1.5 uni s). Using his app oach, 334 pa icipan s aged 21 o 67 yea s we e included (Table 1). O e all, 114 (34%) o he 334 pe sons used some medica ion (Table 2). Fo y emale pa icipan s used sys emic es ogen, p oges in, o hei combina ion ( o con acep ion o ho mone eplacemen he apy), and 1 pa icipan used ibolone. One pa icipan who was physically well and symp omless was ea ed wi h wa a in o an iphospholipid synd ome. SVRI was calcula ed om he adial BP signal and ca diac index measu ed by he Ci cMon de ice. LCWI (in kg9m/min pe m 2 ) was calcula ed using ollowing o mula: 0.01439(mean ao ic p essu e–pulmona y a e y occlusion p essu e)9ca diac index. Pulmona y a e y occlusion p essu e was assumed o be 6 mm Hg (no mal), and 0.0143 is he con e sion ac o o p essu e om millime e s o me cu y o cen ime e s o wa e , olume o densi y o blood (in kg/L), and cen ime e s o me e s. 36,37 The s oke olume and ca diac ou pu measu ed using Ci cMon whole-body impedance ca diog aphy ag ee wi h alues mea- su ed using 3-dimensional ul asound and he he modilu ion me hod, espec i ely, and ag eemen wi h he la e has been shown in bo h supine and up igh posi ions. 34,36,38 An in oduc o y head-up il was pe o med be o e he eco dings. The ac ual measu emen consis ed o 3 consec- u i e 5-minu e pe iods wi h con inuous cap u e o da a: 5 minu es supine, 5 minu es o passi e head-up il o 60°, and 5 minu es supine. The epea abili y and ep oducibili y o he supine and up igh measu emen s has been shown o be good. 31 E alua ion o Ca diac Au onomic Tone HRV analysis om elec oca diog ams was used o assess ca diac au onomic one. The elec oca diog ams we e eco ded by he Ci cMon de ice a a 200-Hz sampling a e, and da a we e analyzed using Ma lab so wa e (Ma hWo ks Inc). No mal R-R in e als we e ecognized, and a bea was conside ed ec opic i he in e al di e ed >20% om he p e ious alues. The a i ac s we e p ocessed using he cubic spline in e pola ion me hod. 39 Because he da a we e collec ed om sho - e m eco dings, he equency domain me hod was applied. 40 The ollowing a iables we e calcula ed om he eco dings in supine (0–5 minu es) and up igh (5–10 minu es) posi ions using he as Fou ie ans o ma- ion me hod: (1) o al powe , (2) powe in he low- equency (LF) ange (0.04–0.15 Hz), (3) powe in he high- equency (HF) ange (0.15–0.40 Hz), and (4) LF/HF a io. 40 Labo a o y Tes s Blood samples we e ob ained a e abou 12 hou s o as ing. Ho mann-La Roche L d), and blood cell coun was pe o med by an ADVIA 120 o 2120 sys em (Siemens Heal hca e GmbH). S a is ical Analyses The cha ac e is ics o men e sus women we e compa ed using an independen samples es , and smoking habi s (cu en , p e ious and ne e ) we e compa ed using he Pea son chi-squa e es . Hemodynamic di e ences in supine and up igh posi ions we e examined using ANOVA o epea ed measu es, and he changes in hemodynamic alues om supine o up igh posi ion (a e age alues o las 3 minu es be o e and las 3 minu es du ing he head-up il 25 ) and HRV di e ences we e analyzed using an independen samples es and ANOVA wi h possible con ounding ac o s as co a ia es. A e age BP alues du ing he las 3 minu es o he fi s supine pe iod we e applied o he clinical cha ac- e iza ion o he BP o pa icipan s (Table 1). The analyses we e adjus ed o smoking habi s, alcohol in ake (s anda d doses pe week), as ing plasma glucose, iglyce ides, high- and low-densi y lipop o ein choles e ol, heigh , and mean a e ial p essu e, as app op ia e ( he adjus ing a iable was no used i i was included in he o mula o he a iable o in e es ). HRV analyses we e also adjus ed o hea a e. 42 In he adjus ed analyses, cu en smoking as ciga e es pe day was applied as a con inuous a iable, whe eas in he ables, he smoking habi s we e desc ibed as cu en , p e ious, and ne e smoke . Due o missing da a among he main g oup o in e es (334 pa icipan s), he numbe o pa icipan s in he adjus ed analyses anged om 311 o 324, wi h a leas 153 men and 158 women in all analyses. The skewed dis ibu ions o iglyce ides, o al powe , LF powe , HF powe , and LF/HF a io we e loga i hmically ans o med be o e s a is ical analyses. Va iables wi h no mal dis ibu ion we e epo ed as means and s anda d de ia ions (SD) o 95% CIs o he means; skewed dis ibu ions we e epo ed as medians, lowe and uppe qua iles, and ange; and ca ego ical a iables we e epo ed as numbe s o pa icipan s and pe cen ages. Men, howe e , we e cha ac e ized by somewha highe sys olic and dias olic BP, alcohol in ake, and highe as ing plasma c ea inine, cys a in C, low-densi y lipop o ein choles e ol, iglyce ide, and glucose concen a ion and lowe high-densi y lipop o ein choles e ol concen a ion han women. Supine and Up igh Hemodynamics Men had highe supine mean a e ial p essu e, s oke index, and LCWI and lowe hea a e han women in unadjus ed analyses (Figu e 1A–1C and 1E). Supine ca diac index and SVRI did no di e be ween sexes (Figu e 1D and 1F). A e adjus ing o heigh , smoking habi s, alcohol in ake, mean a e ial p essu e, low- and high-densi y lipop o ein choles- e ol, iglyce ides, and glucose, only he di e ences in supine hea a e and s oke index o men and women emained significan (Figu e 1B and 1C). Du ing passi e head-up il o 60°, men had highe mean a e ial p essu e, s oke index, ca diac index, and LCWI (Figu e 1A, 1C, 1D, and 1E) and lowe SVRI (Figu e 1F) han women in unadjus ed analyses. Up igh hea a e did no di e be ween men and women (Figu e 1B). In adjus ed analyses, wi h he excep ion o mean a e ial p essu e, all o he abo e di e ences in up igh hemody- namics o men and women emained significan (Figu e 1A and 1C–1F). The magni ude o he changes in hemodynamic a iables in esponse o up igh pos u e was also analyzed (see Me hods). Unadjus ed analyses showed a g ea e inc ease in hea a e (P<0.001); less dec ease in s oke index, ca diac index, and LCWI (P<0.05 o all); and less inc ease in SVRI (P<0.001) in esponse o head-up il in men han in women. A simila inc ease in mean a e ial p essu e was obse ed in bo h sexes (P=0.812). In adjus ed analyses, he di e ences in he up igh changes in ca diac index, LCWI, and SVRI emained significan (P<0.01 o all), whe eas he changes in hea a e and s oke index we e no longe di e en be ween men and women (P=0.600 and P=0.694, espec i ely).
